Description
Overview of the medicine
Amoxicillin is a penicillin antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
250 mg to 500 mg every 8 hours, or 500 mg to 875 mg every 12 hours, depending on the severity and type of infection. For severe infections, doses may be increased.
Elderly
No specific dosage adjustment is usually required in the elderly unless there is significant renal impairment.
Renal_impairment
Dosage adjustment is necessary based on creatinine clearance. For severe renal impairment, the dose and/or frequency should be reduced.
How to Take
Take the capsule orally with a glass of water, with or without food. It is important to complete the full course of treatment as prescribed by your doctor, even if you feel better.
Mechanism of Action
Amoxicillin is a bactericidal agent that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) on the bacterial cell wall, leading to cell lysis and death.
Pharmacokinetics
Onset
Antibacterial effect typically observed within 1 hour.
Excretion
Primarily eliminated via the kidneys, mainly as unchanged drug, through glomerular filtration and tubular secretion.
Half life
Approximately 1 to 1.5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal (GI) tract. Peak plasma concentrations are typically achieved within 1 to 2 hours after oral administration.
Metabolism
Partially metabolized; about 10-25% is metabolized to inactive penicilloic acid.
Side Effects
Contraindications
- Known hypersensitivity to penicillins or any other beta-lactam antibiotics (e.g., cephalosporins)
- Infectious mononucleosis (increased risk of rash)
Drug Interactions
Allopurinol
Concurrent use may increase the incidence of skin rash.
Methotrexate
May decrease methotrexate clearance, leading to increased toxicity.
Oral contraceptives
May reduce the efficacy of oral contraceptives; additional contraceptive methods may be needed.
Warfarin and other oral anticoagulants
May prolong prothrombin time (INR); careful monitoring is required.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and electrolyte imbalance. In cases of massive overdose, renal failure and convulsions may occur. Treatment involves symptomatic and supportive measures; gastric lavage may be considered if ingestion was recent. Hemodialysis can remove amoxicillin from circulation.
Pregnancy & Lactation
Pregnancy Category B. Amoxicillin is generally considered safe for use during pregnancy, though it should be used with caution and only if clearly needed. Small amounts are excreted in breast milk; generally considered compatible with breastfeeding, but monitor the infant for diarrhea or candidiasis.

Lab Monitoring
- Renal function tests (e.g., serum creatinine, BUN) during prolonged therapy, especially in patients with pre-existing renal impairment.
- Complete blood count (CBC) with differential during prolonged therapy.
Doctor Notes
- Emphasize the importance of completing the full prescribed course to prevent resistance.
- Advise patients to report any signs of allergic reaction immediately.
- Caution about potential interaction with oral contraceptives and warfarin.
