Description
Overview of the medicine
Ultra D is a brand of Cholecalciferol (Vitamin D3) supplement, used to prevent and treat Vitamin D deficiency and associated conditions. It plays a crucial role in calcium absorption and bone health.
Uses & Indications
Dosage
Adults
Typically 20,000 IU once weekly or 40,000 IU every two weeks for deficiency, or lower doses for maintenance (e.g., 800-2000 IU daily).
Elderly
Similar to adults, but may require higher doses due to reduced skin synthesis and dietary intake.
Renal_impairment
Use with caution; active form (calcitriol) might be preferred in severe impairment.
How to Take
To be taken orally, preferably with the largest meal of the day to optimize absorption.
Mechanism of Action
Cholecalciferol is converted in the liver to 25-hydroxycholecalciferol (calcidiol) and then in the kidneys to 1,25-dihydroxycholecalciferol (calcitriol), the active form of Vitamin D. Calcitriol promotes calcium and phosphate absorption from the gut, regulates bone remodeling, and modulates immune function.
Pharmacokinetics
Onset
Days to weeks for significant therapeutic effects.
Excretion
Mainly through bile into feces; a small amount is excreted in urine.
Half life
Variable, active metabolites have a longer half-life (e.g., calcifediol ~15 days, calcitriol ~4-6 hours).
Absorption
Readily absorbed from the gastrointestinal tract, especially in the presence of dietary fat.
Metabolism
Primarily hepatic hydroxylation to 25-hydroxycholecalciferol (calcidiol), then renal hydroxylation to 1,25-dihydroxycholecalciferol (calcitriol).
Side Effects
Contraindications
- •Hypercalcemia (high calcium levels).
- •Vitamin D toxicity.
- •Hypersensitivity to Cholecalciferol or any components.
Drug Interactions
Corticosteroids
May reduce Vitamin D effects.
Thiazide diuretics
May increase risk of hypercalcemia.
Anticonvulsants (e.g., phenytoin, barbiturates)
May increase Vitamin D metabolism.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include hypercalcemia (nausea, vomiting, weakness, polyuria, constipation), renal impairment. Management involves discontinuing Vitamin D, low-calcium diet, and hydration. Severe cases may require corticosteroids or diuretics.
Pregnancy & Lactation
Safe at recommended doses, but excessive doses should be avoided. Consult a doctor.
