Nitrofurantoin is reduced by bacterial flavoproteins to reactive intermediates that inactivate or damage bacterial ribosomal proteins, DNA, RNA, and other macromolecules. This leads to the inhibition of bacterial cell wall synthesis, DNA replication, RNA synthesis, and protein synthesis.

• •Severe renal impairment (creatinine clearance < 60 mL/min)
• •G6PD deficiency
• •Infants younger than 1 month
• •Late-term pregnancy (38-42 weeks gestation or during labor and delivery)
• •History of cholestatic jaundice or hepatic dysfunction with nitrofurantoin

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Overdose may lead to nausea, vomiting, abdominal pain. Management involves gastric lavage, supportive care, and symptomatic treatment. Increased fluid intake can promote urinary excretion.

Pregnancy Category B (avoid in late term due to potential for hemolytic anemia in the neonate). Excreted in breast milk, generally considered compatible with breastfeeding, but avoid if infant has G6PD deficiency or is < 1 month old.
