Overview of the medicine
Urobak (Nitrofurantoin) is an antibiotic used to treat and prevent urinary tract infections (UTIs) caused by susceptible bacteria.
Uses & Indications
Dosage
Adults
For treatment: 50-100 mg four times daily for 7 days. For prophylaxis: 50-100 mg once daily at bedtime.
Elderly
Similar to adults, but with caution and monitoring of renal function due to increased risk of side effects.
Renal_impairment
Contraindicated in severe renal impairment (creatinine clearance < 60 mL/min).
How to Take
Take Urobak with food or milk to enhance absorption and minimize gastrointestinal upset. Swallow capsules whole.
Mechanism of Action
Nitrofurantoin is reduced by bacterial flavoproteins to reactive intermediates that inactivate or damage bacterial ribosomal proteins, DNA, RNA, and other macromolecules. This leads to the inhibition of bacterial cell wall synthesis, DNA replication, RNA synthesis, and protein synthesis.

Side Effects
Contraindications
- •Severe renal impairment (creatinine clearance < 60 mL/min)

- •History of cholestatic jaundice or hepatic dysfunction with nitrofurantoin
Drug Interactions
Probenecid
Reduces renal excretion of nitrofurantoin, increasing systemic levels and risk of toxicity.
Antacids (containing magnesium trisilicate)
May reduce absorption of nitrofurantoin.
Live bacterial vaccines (e.g., typhoid vaccine)
May decrease vaccine efficacy.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to nausea, vomiting, abdominal pain. Management involves gastric lavage, supportive care, and symptomatic treatment. Increased fluid intake can promote urinary excretion.
Pregnancy & Lactation
Pregnancy Category B (avoid in late term due to potential for hemolytic anemia in the neonate). Excreted in breast milk, generally considered compatible with breastfeeding, but avoid if infant has G6PD deficiency or is < 1 month old.
