Description
Overview of the medicine
Velofix 200 mg Capsule contains Cefixime, which is a third-generation cephalosporin antibiotic used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
200 mg once daily or 100 mg twice daily. For severe infections, 200 mg twice daily.
Elderly
No dosage adjustment is generally required unless renal function is significantly impaired.
Renal_impairment
For creatinine clearance 21-60 mL/min, dosage should be 75% of the standard dose. For creatinine clearance <20 mL/min, dosage should be 50% of the standard dose.
How to Take
Take orally with or without food. To minimize gastrointestinal upset, it can be taken with food.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis, leading to bactericidal activity. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, preventing the final transpeptidation step of peptidoglycan synthesis.
Pharmacokinetics
Onset
Within 1 hour (for antibacterial effect)
Excretion
Primarily excreted unchanged in the urine (about 50%) and bile (about 10%).
Half life
Approximately 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food can slightly delay absorption but does not significantly affect the extent of absorption.
Metabolism
Not extensively metabolized; some formation of inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Cefixime or any other cephalosporin antibiotics
- History of serious hypersensitivity reactions to penicillin or other beta-lactam antibiotics
Drug Interactions
Warfarin
May increase prothrombin time, monitor INR closely.
Probenecid
May increase Cefixime plasma levels by decreasing renal excretion.
Carbamazepine
Concomitant administration has been reported to increase plasma carbamazepine levels.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Cefixime is excreted in small amounts into breast milk; caution should be exercised when administered to a nursing woman.
