Verizith 200 mg/5ml Suspension is a macrolide antibiotic used to treat a wide range of bacterial infections including respiratory tract infections, skin infections, and certain sexually transmitted diseases.
Uses & Indications
Dosage
Adults
The dosage for adults depends on the infection. For example, for uncomplicated Chlamydia trachomatis, a single 1000 mg dose is given. For other infections, 500 mg once daily for 3 days or 500 mg on day 1, then 250 mg once daily for 4 days. (Note: Suspension form less common for adults but can be used).
Elderly
No specific dosage adjustment is needed for elderly patients with normal renal and hepatic function. Dosage should be similar to adults.
Pediatrics
For pediatric patients (6 months and older) with acute otitis media, 30 mg/kg as a single dose OR 10 mg/kg once daily for 3 days OR 10 mg/kg on day 1 followed by 5 mg/kg for 4 days. For pharyngitis/tonsillitis, 12 mg/kg once daily for 5 days.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment (creatinine clearance >10 mL/min). Use with caution in severe renal impairment.
Hepatic_impairment
Use with caution in patients with hepatic impairment, as Azithromycin is primarily metabolized and excreted by the liver. Dosage reduction may be considered.
How to Take
Administer orally, with or without food. For best absorption, it's often recommended to take it an hour before or two hours after a meal. Ensure the suspension is well-shaken before each use. Use the provided measuring device for accurate dosing.
Mechanism of Action
Azithromycin works by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby inhibiting bacterial protein synthesis. This results in bacteriostatic action, preventing bacterial growth.

Excretion
Mainly excreted unchanged in bile. A small percentage is excreted in urine.
Half life
Terminal elimination half-life is 2-4 days (average 68 hours) due to extensive tissue uptake and slow release.
Absorption
Rapidly absorbed from the gastrointestinal tract; however, bioavailability is approximately 37% due to extensive first-pass metabolism. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Primarily metabolized in the liver by N-demethylation.
Side Effects
Contraindications
- •Hypersensitivity to Azithromycin, Erythromycin, or any macrolide or ketolide antibiotic.
- •History of cholestatic jaundice/hepatic dysfunction associated with prior Azithromycin use.
Drug Interactions
Digoxin
Increased Digoxin concentrations. Monitor Digoxin levels.
Warfarin
Increased anticoagulant effect; monitor PT/INR closely.
Nelfinavir
Increased Azithromycin concentrations. Monitor for known side effects.
Ergot Alkaloids (e.g., Ergotamine)
Potential for acute ergotism. Avoid concomitant use.
Antacids containing aluminum or magnesium
May decrease peak plasma levels of Azithromycin. Administer Azithromycin at least 1 hour before or 2 hours after antacids.
Storage
Store dry powder below 30°C, protected from light and moisture. Store reconstituted suspension at room temperature (20-25°C) and discard any unused portion after 10 days. Do not freeze.
Overdose
Symptoms of Azithromycin overdose include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves symptomatic and supportive measures. Gastric lavage and general supportive care are recommended.
Pregnancy & Lactation
Pregnancy Category B. Azithromycin should be used during pregnancy only if clearly needed. It is excreted in human milk; caution should be exercised when administered to a nursing woman.
