Azithromycin is a macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin infections, ear infections, and sexually transmitted diseases.
Uses & Indications
Dosage
Adults
Typical dosage is 250-500 mg once daily for 3-5 days, or a single 1g dose depending on the infection. For Chlamydia: 1g single dose. For MAC: 600 mg once daily.
Elderly
No specific dosage adjustment is required, but monitor for adverse cardiac events due to QT prolongation risk.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment (CrCl >10 mL/min). Caution is advised in severe renal impairment (CrCl <10 mL/min).
How to Take
Verizith capsules can be taken orally with or without food. To maximize absorption and minimize gastrointestinal upset, some prefer to take it with food.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby preventing bacterial growth and replication.

Onset
Peak plasma concentrations reached within 2-3 hours.
Excretion
Mainly excreted unchanged in bile; a small proportion is excreted renally.
Half life
Terminal elimination half-life is 2-4 days.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%.
Metabolism
Primarily hepatic, but extensive metabolism is not observed. Major routes involve demethylation.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or to any component of the formulation.
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
Macrolides can increase digoxin levels. Monitor digoxin levels if co-administered.
Warfarin
Increased anticoagulant effect has been reported. Monitor INR closely.
Nelfinavir
Increased serum concentrations of azithromycin. No dosage adjustment is needed, but monitor for known adverse effects of azithromycin.
Antacids (containing aluminum or magnesium)
May decrease the peak plasma concentration of azithromycin. Administer azithromycin at least 2 hours before or 4 hours after antacids.
Ergot alkaloids (e.g., ergotamine, dihydroergotamine)
Potential for acute ergotism, characterized by severe peripheral vasoconstriction and dysesthesia. Concomitant use is contraindicated.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves gastric lavage and general supportive measures as clinically indicated.
Pregnancy & Lactation
Pregnancy Category B. Azithromycin should be used during pregnancy only if clearly needed. It is excreted in human breast milk; caution should be exercised when administered to a nursing woman.
