Vigorex 50 mg Tablet contains Sildenafil, a phosphodiesterase-5 (PDE5) inhibitor, used for the treatment of erectile dysfunction (ED) in men. It helps to achieve and maintain an erection sufficient for satisfactory sexual activity.
Uses & Indications
Dosage
Adults
The recommended dose is 50 mg taken approximately 1 hour before sexual activity. Based on efficacy and tolerability, the dose may be increased to a maximum of 100 mg or decreased to 25 mg. The maximum recommended dosing frequency is once per day.
Elderly
A starting dose of 25 mg should be considered in elderly patients (≥65 years) due to reduced clearance and increased plasma levels.
Renal_impairment
For patients with severe renal impairment (creatinine clearance <30 mL/min), a starting dose of 25 mg should be considered.
How to Take
Take Vigorex 50 mg Tablet orally approximately 1 hour before sexual activity. It can be taken with or without food. However, a high-fat meal may delay the onset of action.
Mechanism of Action
Sildenafil inhibits phosphodiesterase type 5 (PDE5), which is responsible for the degradation of cyclic guanosine monophosphate (cGMP) in the corpus cavernosum of the penis. By inhibiting PDE5, Sildenafil increases cGMP levels, leading to relaxation of smooth muscles and increased blood flow into the penis, resulting in an erection. It requires sexual stimulation to be effective.

Metabolism
Primarily metabolized by hepatic microsomal isoenzymes CYP3A4 (major route) and CYP2C9 (minor route). The major circulating metabolite, N-desmethyl sildenafil, has about 50% of the potency of the parent drug.
Side Effects
Contraindications
- Concomitant use with nitrates (e.g., nitroglycerin, isosorbide dinitrate) in any form, as it may cause a profound drop in blood pressure.
- Concomitant use with guanylate cyclase stimulators (e.g., riociguat).
- Hypersensitivity to Sildenafil or any component of the tablet.
- Patients with severe cardiovascular events (e.g., recent myocardial infarction or stroke).
- Severe hepatic impairment.
- Hypotension (blood pressure <90/50 mmHg).
- Known hereditary degenerative retinal disorders (e.g., retinitis pigmentosa).
Drug Interactions
Nitrates
Contraindicated. Potentiates the hypotensive effect of nitrates, leading to severe hypotension.
Riociguat
Contraindicated. Concomitant use may cause symptomatic hypotension.
Alpha-blockers
Risk of symptomatic hypotension due to additive vasodilatory effects. Initiate Sildenafil at 25 mg with caution in patients taking alpha-blockers.
CYP3A4 Inhibitors (e.g., Ritonavir, Ketoconazole, Erythromycin)
May significantly increase Sildenafil plasma levels. A lower starting dose (25 mg) of Sildenafil is recommended when co-administered.

Overdose
In studies with healthy volunteers, single doses up to 800 mg have been administered, resulting in adverse events similar to those seen at lower doses but generally more pronounced. In cases of overdose, standard supportive measures should be adopted as required. Renal dialysis is not expected to accelerate clearance as Sildenafil is highly bound to plasma proteins and not significantly excreted in the urine.
