Description
Overview of the medicine
Visral 20 mg Suppository contains Bisacodyl, a stimulant laxative used for the short-term relief of occasional constipation and for bowel preparation before medical procedures.
Uses & Indications
Dosage
Adults
For constipation, 1 suppository (20 mg) rectally once daily. For bowel preparation, consult physician for specific regimen.
Elderly
Same as adults, but caution should be exercised to avoid dehydration and electrolyte imbalance.
Renal_impairment
No specific dose adjustment required, but use with caution in severe renal impairment due to potential for electrolyte disturbances.
How to Take
Insert one suppository into the rectum, pointed end first. It is usually effective within 15-60 minutes.
Mechanism of Action
Bisacodyl acts by stimulating the nerve endings in the colon, which increases peristaltic contractions and promotes the accumulation of water and electrolytes in the colonic lumen, leading to laxation.
Pharmacokinetics
Onset
15-60 minutes after rectal administration.
Excretion
Mainly via feces. A small amount of absorbed drug is excreted renally.
Half life
Not clinically significant for rectal administration due to local action.
Absorption
Minimal systemic absorption (less than 5%) occurs after rectal administration, as the drug primarily acts locally in the colon.
Metabolism
Undergoes enterohepatic circulation after hydrolysis by colonic bacteria to its active form, but systemic metabolism is minimal.
Side Effects
Contraindications
- Acute abdominal conditions (e.g., appendicitis, acute inflammatory bowel diseases)
- Severe dehydration
- Intestinal obstruction or paralytic ileus
- Undiagnosed abdominal pain
- Children under 10 years without medical advice
Drug Interactions
Diuretics and Corticosteroids
May increase the risk of electrolyte imbalance (especially hypokalemia) with excessive use of Bisacodyl.
Antacids, milk, proton pump inhibitors
Should not be taken within one hour of oral Bisacodyl due to premature dissolution of enteric coating; less relevant for suppositories.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe abdominal cramps, diarrhea, and significant fluid and electrolyte loss, particularly potassium. Treatment is supportive, focusing on fluid and electrolyte replacement.
Pregnancy & Lactation
Generally considered safe for short-term use during pregnancy and lactation, but always consult a healthcare professional before use.

Clinical Trials
As Bisacodyl is a well-established and widely used laxative, extensive clinical data supports its efficacy and safety. Specific trials for a 20mg suppository formulation are mostly related to its use in bowel preparation.
Lab Monitoring
- Routine lab monitoring is not required for short-term use. For chronic or excessive use, monitoring of serum electrolytes (e.g., potassium) may be advisable.
Doctor Notes
- Emphasize short-term use and discourage chronic use without medical supervision.
- Educate patients on lifestyle modifications for constipation management.
- Monitor for electrolyte imbalance in patients with prolonged use or underlying conditions.
Patient Guidelines
- Use for short-term relief of constipation only.
- Do not use for more than 7 days unless directed by a doctor.
- Drink plenty of fluids to prevent dehydration.
- If constipation persists, consult your doctor.
Missed Dose Advice
Since Bisacodyl suppositories are used on an 'as needed' basis for constipation, a missed dose is not typically a concern. Do not double the dose to make up for a missed one.
Driving Precautions
Visral 20 mg Suppository is not known to affect the ability to drive or operate machinery.
Lifestyle Advice
- Maintain a diet rich in fiber (fruits, vegetables, whole grains).
- Ensure adequate fluid intake throughout the day.
- Incorporate regular physical activity into your routine.
