Overview of the medicine
Vistarin-4000 anti-Xa Injection contains Enoxaparin Sodium, a low molecular weight heparin (LMWH). It is used to prevent and treat various thromboembolic disorders by thinning the blood and preventing blood clot formation.
Uses & Indications
Dosage
Adults
Varies based on indication: e.g., DVT prophylaxis: 40 mg (4000 anti-Xa IU) once daily SC; DVT/PE treatment: 1 mg/kg (100 anti-Xa IU/kg) every 12 hours SC.
Elderly
No dosage adjustment needed unless severe renal impairment. For patients >75 years, caution is advised for bleeding risk.
Renal_impairment
Severe renal impairment (CrCl < 30 mL/min): DVT prophylaxis: 20 mg (2000 anti-Xa IU) once daily SC; DVT/PE treatment: 1 mg/kg (100 anti-Xa IU/kg) once daily SC.
How to Take
Administer by deep subcutaneous injection, preferably into the abdominal wall. Do not inject intramuscularly. Alternate injection sites. Do not expel the air bubble from the syringe before injection to avoid loss of drug.
Mechanism of Action
Enoxaparin works by binding to and potentiating the activity of antithrombin III. This leads to the inactivation of Factor Xa and, to a lesser extent, Factor IIa (thrombin), thereby preventing the formation of new clots and the extension of existing ones.

Onset
Within 3-5 hours (peak anti-Xa activity after SC injection).
Excretion
Renal excretion of active and inactive fragments; approximately 40% of anti-Xa activity is excreted in urine within 24 hours.
Half life
Approximately 4-5 hours (anti-Xa activity) after a single SC dose.
Absorption
Rapid and complete absorption after subcutaneous (SC) administration, with peak anti-Xa activity occurring 3-5 hours post-dose.
Metabolism
Primarily metabolized in the liver by desulfation and depolymerization to lower molecular weight species with reduced biological potency.
Side Effects
Contraindications
- •Active major bleeding
- •Hypersensitivity to enoxaparin, heparin, or porcine products
- •History of heparin-induced thrombocytopenia (HIT) with or without thrombosis
- •Acute bacterial endocarditis
- •Spinal or epidural anesthesia or lumbar puncture in patients on therapeutic enoxaparin due to risk of spinal hematoma
Drug Interactions
Dextran
Increased risk of bleeding.
Warfarin
Increased risk of bleeding. Close monitoring of INR is required when transitioning.
NSAIDs (e.g., Aspirin, Ibuprofen)
Increased risk of bleeding due to antiplatelet effects.
Platelet inhibitors (e.g., Clopidogrel)
Increased risk of bleeding.
Other anticoagulants (e.g., Dabigatran, Rivaroxaban)
Increased risk of bleeding. Concomitant use is generally not recommended.
Storage
Store below 25°C. Do not freeze. Protect from light.
Overdose
Overdose may lead to hemorrhagic complications. The anticoagulant effects can be largely neutralized by the slow intravenous injection of protamine sulfate. Dosage of protamine depends on the enoxaparin dose and time since administration.
Pregnancy & Lactation
Pregnancy Category B. Can be used during pregnancy when clearly needed, as it does not cross the placenta. Caution advised during lactation; consult a healthcare professional.
