Description
Overview of the medicine
Vistarin 8000 anti-Xa Injection is an anticoagulant medication containing a Factor Xa inhibitor, used to prevent and treat various thromboembolic disorders. It is administered via subcutaneous injection.
Uses & Indications
Dosage
Adults
For VTE prophylaxis: 4000 anti-Xa units subcutaneously once daily. For DVT/PE treatment: 8000 anti-Xa units subcutaneously twice daily or 16000 anti-Xa units once daily.
Elderly
No dose adjustment generally required based on age alone, but monitor renal function.
Renal_impairment
Consider dose reduction in severe renal impairment (CrCl <30 mL/min). Close monitoring of anti-Xa levels is recommended.
How to Take
Administer by deep subcutaneous injection. Do not inject intramuscularly. Alternate injection sites (e.g., abdominal wall, thigh). Do not rub the injection site after administration.
Mechanism of Action
Vistarin selectively binds to and inhibits Factor Xa, an enzyme central to the coagulation cascade. By inhibiting Factor Xa, it prevents the conversion of prothrombin to thrombin, thereby reducing thrombus formation and propagation.
Pharmacokinetics
Onset
1-2 hours after subcutaneous injection.
Excretion
Primarily excreted renally (approximately 40% as unchanged drug and metabolites).
Half life
Approximately 4-7 hours for anti-Xa activity, dose-dependent.
Absorption
Rapid and complete absorption after subcutaneous administration, reaching peak plasma levels within 3-5 hours.
Metabolism
Partially metabolized in the liver, primarily via desulfation and depolymerization.
Side Effects
Contraindications
- •Active major bleeding
- •History of heparin-induced thrombocytopenia (HIT)
- •Hypersensitivity to the active substance or any excipients
- •Acute bacterial endocarditis
Drug Interactions
Dextran
Increased risk of bleeding.
Warfarin
Increased risk of bleeding; monitor INR.
Other Anticoagulants (e.g., DOACs)
Concomitant use may increase bleeding risk; generally avoided or used with extreme caution.
NSAIDs, Aspirin, Antiplatelet agents
Increased risk of bleeding; use with caution.
Storage
Store below 25°C. Do not freeze. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include bleeding. Management involves discontinuing Vistarin and managing bleeding complications. Protamine sulfate may be used to neutralize anti-Factor Xa activity in severe cases, though its effect on LMWHs is partial.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. LMWHs do not cross the placenta. Caution advised during lactation; consult physician.
