Overview of the medicine
Wormex 400 mg Chewable Tablet contains Albendazole, an anthelmintic medication used to treat various parasitic worm infections in humans.
Uses & Indications
Dosage
Adults
For common intestinal worms: 400 mg as a single dose. For specific infections like Neurocysticercosis or Hydatid disease, dosage and duration vary and are determined by a physician.
Elderly
No specific dose adjustment is generally required in the elderly, but caution is advised in patients with hepatic impairment.
Renal_impairment
No specific dose adjustment is necessary as renal excretion is minimal. However, caution is advised in severe renal impairment.
How to Take
This tablet should be chewed or crushed before swallowing. It can be taken with or without food, but taking it with a fatty meal can increase its absorption and effectiveness.
Mechanism of Action
Albendazole selectively binds to beta-tubulin in intestinal and other tissue helminths, inhibiting microtubule polymerization. This leads to impaired glucose uptake and depletion of glycogen stores in the parasite, ultimately resulting in its immobilization and death.
Pharmacokinetics
Onset
Peak plasma concentrations of the active metabolite (albendazole sulfoxide) are reached within 2-5 hours.
Excretion
Mainly excreted in the bile as the sulfoxide metabolite, with less than 1% excreted renally.
Half life
The plasma half-life of albendazole sulfoxide is approximately 8-12 hours.
Absorption
Poorly absorbed from the gastrointestinal tract (less than 5%). Absorption is significantly increased when taken with a fatty meal.
Metabolism
Rapidly and extensively metabolized in the liver to its primary active metabolite, albendazole sulfoxide, and then to albendazole sulfone.
Side Effects
Contraindications
- •Hypersensitivity to Albendazole or any component of the tablet.
- •Pregnancy, especially during the first trimester, due to potential embryotoxic and teratogenic effects.
- •Severe hepatic impairment.
Drug Interactions
Theophylline
Albendazole may decrease the plasma concentration of Theophylline, requiring monitoring of Theophylline levels.
Cimetidine, Dexamethasone, Praziquantel
These drugs can increase plasma concentrations of albendazole sulfoxide, potentially leading to increased side effects.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms are likely to be an exaggeration of known side effects, such as gastrointestinal upset, dizziness, and headache. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Albendazole is contraindicated in the first trimester of pregnancy. For subsequent trimesters, it should only be used if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk in small amounts; use during lactation should be with caution and only if clearly needed.
