Description
Overview of the medicine
Albendazole is an anthelmintic medication used to treat various parasitic worm infections. It works by preventing the worms from absorbing sugar (glucose), which causes them to lose energy and die.
Uses & Indications
Dosage
Adults
For most common intestinal worms (Ascaris, Hookworm, Pinworm, Whipworm): 400 mg (equivalent to 40 ml of 50 mg/5 ml suspension) as a single dose. For Strongyloidiasis or Taeniasis: 400 mg (40 ml) once daily for 3 days. For Hydatid disease or Neurocysticercosis: 400 mg (40 ml) twice daily with meals for 8-30 days, or longer depending on the condition.
Elderly
Dosage adjustment is generally not required for elderly patients unless there is significant hepatic or renal impairment.
Renal_impairment
No specific dose adjustment is considered necessary for patients with renal impairment, but caution is advised in severe cases.
How to Take
Wormex 50 mg/5 ml Suspension is for oral administration. Shake the bottle well before each use. It should be taken with food, especially fatty meals, to enhance absorption.
Mechanism of Action
Albendazole binds to beta-tubulin, inhibiting microtubule polymerization in parasitic helminths. This impairs glucose uptake, leading to glycogen depletion, reduced ATP production, and eventually worm immobilization and death.
Pharmacokinetics
Onset
Onset of action is variable, depending on the type of worm and infection site. Therapeutic effects typically manifest within a few days.
Excretion
Albendazole and its metabolites are primarily excreted via bile and, to a lesser extent, urine.
Half life
The plasma half-life of albendazole sulfoxide (the active metabolite) is approximately 8-12 hours.
Absorption
Poorly absorbed from the gastrointestinal tract (<5%). Absorption is significantly increased when taken with fatty meals.
Metabolism
Extensively undergoes first-pass metabolism in the liver to its primary active metabolite, albendazole sulfoxide, which is then further metabolized to inactive sulfone.
Side Effects
Contraindications
- •Hypersensitivity to albendazole or other benzimidazole derivatives.
- •Pregnancy (especially during the first trimester).
- •Children under 1 year of age for certain indications (e.g., strongyloidiasis, hydatid disease).
Drug Interactions
Ritonavir
May reduce the exposure of albendazole sulfoxide.
Cimetidine
Increases plasma concentrations of albendazole sulfoxide.
Praziquantel
Increases plasma concentrations of albendazole sulfoxide.
Theophylline
Albendazole may decrease theophylline clearance; monitor theophylline levels.
Dexamethasone
Increases plasma concentrations of albendazole sulfoxide.
Storage
Store below 30°C. Protect from direct light and moisture. Keep out of reach of children. Do not freeze.
Overdose
Symptoms of albendazole overdose may include gastrointestinal disturbances (e.g., nausea, vomiting, abdominal pain), dizziness, lethargy, and transient liver enzyme elevations. Treatment is symptomatic and supportive, including gastric lavage or activated charcoal if ingestion was recent.
Pregnancy & Lactation
Pregnancy: Albendazole is contraindicated in the first trimester of pregnancy due to potential teratogenicity. In later trimesters, it should only be used if the potential benefit justifies the potential risk to the fetus. It is classified as Pregnancy Category C. Lactation: Small amounts of albendazole metabolites may be excreted in breast milk. Use with caution in breastfeeding mothers; consult a doctor.
