Description
Overview of the medicine
Xadu 100 mg Tablet contains Xaduflosin, a selective sodium-glucose cotransporter 2 (SGLT2) inhibitor indicated for the treatment of type 2 diabetes mellitus.
Uses & Indications
Dosage
Adults
The recommended starting dose is 100 mg once daily, taken orally in the morning, with or without food. The dose may be adjusted based on efficacy and tolerability.
Elderly
No dose adjustment is required based on age alone. However, renal function should be monitored, as elderly patients are more likely to have decreased renal function.
Renal_impairment
Not recommended for patients with eGFR <30 mL/min/1.73 m². For eGFR 30 to <60 mL/min/1.73 m², the dose should not exceed 100 mg once daily.
How to Take
Take Xadu 100 mg Tablet orally once daily in the morning, with or without food. Swallow the tablet whole with water. Do not crush, chew, or break the tablet.
Mechanism of Action
Xaduflosin inhibits SGLT2 in the renal tubules, reducing glucose reabsorption and increasing urinary glucose excretion, thereby lowering blood glucose levels. It also leads to modest reductions in blood pressure and body weight.
Pharmacokinetics
Onset
Glucose-lowering effects observed within 1 hour.
Excretion
Approximately 70% excreted renally (as metabolites and unchanged drug) and 30% via feces.
Half life
Approximately 12-15 hours, supporting once-daily dosing.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations (Tmax) reached within 1-2 hours. Absolute bioavailability is approximately 80%.
Metabolism
Primarily metabolized in the liver via UGT1A9 and UGT2B7 to inactive glucuronide conjugates.
Side Effects
Contraindications
- Known hypersensitivity to Xaduflosin or any excipient in the formulation.
- Patients with severe renal impairment (eGFR <30 mL/min/1.73 m²), end-stage renal disease (ESRD), or on dialysis.
- Patients with type 1 diabetes mellitus (due to increased risk of diabetic ketoacidosis).
Drug Interactions
Digoxin
Xaduflosin may slightly increase digoxin exposure, requiring monitoring.
Diuretics (e.g., Thiazides, Loop diuretics)
May increase the risk of dehydration and hypotension.
Insulin and Insulin Secretagogues (e.g., Sulfonylureas)
Increased risk of hypoglycemia when used concomitantly; dose reduction of insulin or secretagogue may be necessary.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of an overdose, symptomatic and supportive treatment should be initiated. Xaduflosin is not expected to be dialyzable.
Pregnancy & Lactation
Not recommended during pregnancy due to potential fetal renal dysfunction in animal studies. Avoid during lactation as it is unknown if Xaduflosin is excreted in human milk.

Clinical Trials
Phase 3 clinical trials demonstrated significant reduction in HbA1c, fasting plasma glucose, and body weight. Long-term studies also showed cardiovascular and renal benefits.
Lab Monitoring
- Renal function (eGFR) should be assessed prior to initiation and periodically thereafter.
- Blood glucose levels (fasting plasma glucose, HbA1c) should be monitored regularly.
- Electrolyte levels should be monitored in patients at risk of dehydration.
Doctor Notes
- Assess renal function (eGFR) before initiating Xaduflosin and periodically thereafter. Adjust dose or discontinue based on eGFR.
- Counsel patients on the risk of DKA, particularly in situations of acute illness, surgery, or reduced carbohydrate intake.
- Monitor for signs and symptoms of volume depletion and hypotension, especially in elderly patients or those on diuretics.
Patient Guidelines
- Take Xadu 100 mg Tablet exactly as prescribed by your doctor.
- Stay well-hydrated by drinking plenty of fluids, especially in hot weather or during illness.
- Be aware of symptoms of urinary tract infections, genital yeast infections, and diabetic ketoacidosis, and report them to your doctor immediately.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Xaduflosin has no or negligible influence on the ability to drive and use machines. However, patients experiencing symptoms like dizziness or hypoglycemia should exercise caution.
Lifestyle Advice
- Continue to follow a healthy diet and regular exercise program as recommended by your healthcare provider.
- Regularly monitor your blood glucose levels as advised.
