Description
Overview of the medicine
Xadu 70 mg Tablet is a bisphosphonate used for the treatment and prevention of osteoporosis in postmenopausal women and for increasing bone mass in men with osteoporosis. It helps to slow bone loss and may help rebuild bone, reducing the risk of fractures.
Uses & Indications
Dosage
Adults
For osteoporosis treatment: 70 mg once weekly. For osteoporosis prevention: 35 mg once weekly (if available).
Elderly
No dosage adjustment is generally required for elderly patients with normal renal function.
Renal_impairment
Not recommended for patients with creatinine clearance <35 mL/min. For patients with mild to moderate renal impairment, no dosage adjustment is needed, but caution is advised.
How to Take
Take the tablet with a full glass of plain water (not mineral water, coffee, juice, or milk) first thing in the morning, at least 30 minutes before any food, drink, or other medications. Remain upright (sitting or standing) for at least 30 minutes after taking the tablet and until after the first meal of the day to minimize esophageal irritation.
Mechanism of Action
Xadufos acts as a potent, non-hormonal antiresorptive agent. It preferentially localizes to sites of bone resorption, specifically under osteoclasts, and inhibits osteoclast activity, thereby reducing bone resorption without directly affecting bone mineralization.
Pharmacokinetics
Onset
Clinical effects on bone turnover may be observed within weeks; significant increase in BMD typically seen after 6-12 months.
Excretion
Approximately 50% of absorbed drug is excreted unchanged via the kidneys; the remainder is incorporated into bone.
Half life
Plasma half-life is very short (several hours), but the effective half-life in bone can be many years due to strong binding to bone mineral.
Absorption
Poorly absorbed from the GI tract (typically <1%), and absorption is significantly reduced by food and certain medications. Peak plasma concentration achieved within 1 hour.
Metabolism
Not metabolized in humans.
Side Effects
Contraindications
- Abnormalities of the esophagus which delay esophageal emptying (e.g., stricture or achalasia).
- Inability to stand or sit upright for at least 30 minutes.
- Hypocalcemia.
- Hypersensitivity to any component of the product.
Drug Interactions
Calcium supplements/Antacids
Significantly interfere with the absorption of Xadufos. Must be taken at least 30 minutes after Xadufos.
NSAIDs (Non-Steroidal Anti-Inflammatory Drugs)
Concurrent use may increase the risk of upper gastrointestinal adverse events.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Overdose may result in hypocalcemia, hypophosphatemia, and upper gastrointestinal adverse events, such as upset stomach, heartburn, esophagitis, gastritis, or ulcer. Milk or antacids should be given to bind Xadufos. Patients should be observed for signs of hypocalcemia.
Pregnancy & Lactation
Pregnancy Category C. Not recommended for use during pregnancy or lactation due to lack of adequate data and potential for fetal harm. Women of childbearing potential should use effective contraception.
