Description
Overview of the medicine
Xanuric 40 mg Tablet is a medication used to lower high levels of cholesterol and triglycerides in the blood. It helps reduce the risk of heart attack, stroke, and other cardiovascular complications in individuals with hyperlipidemia.
Uses & Indications
Dosage
Adults
Initial dose 10-20 mg once daily, adjusted to 40 mg once daily based on LDL-C response and tolerability. Max dose 80 mg daily.
Elderly
No specific dose adjustment required, but cautious titration from a lower dose (e.g., 10 mg) may be considered.
Renal_impairment
No dose adjustment needed for mild to moderate impairment (CrCl >30 mL/min); use with caution in severe impairment (CrCl <30 mL/min) and monitor closely.
How to Take
Administer orally once daily, with or without food, preferably in the evening to align with the body's natural cholesterol production cycle.
Mechanism of Action
Xanurin Calcium selectively inhibits HMG-CoA reductase, an enzyme that catalyzes the early rate-limiting step in cholesterol biosynthesis. This inhibition decreases hepatic cholesterol synthesis and increases the number of LDL receptors on the hepatocyte surface, leading to increased uptake and catabolism of LDL.
Pharmacokinetics
Onset
Lipid-lowering effects observed within 1 week, maximal effect typically by 2-4 weeks.
Excretion
Mainly excreted in feces via bile; minimal renal excretion (<5%).
Half life
Approximately 14-19 hours.
Absorption
Rapidly absorbed after oral administration; bioavailability approximately 30-50%. Peak plasma concentrations are reached within 2-4 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4, to active and inactive metabolites.
Side Effects
Contraindications
- Active liver disease or unexplained persistent elevations of serum transaminases
- Pregnancy and lactation
- Hypersensitivity to Xanurin or any component of the formulation
- Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin) at high Xanuric doses
Drug Interactions
Warfarin
Potentiates anticoagulant effect; monitor International Normalized Ratio (INR) closely at initiation and dose changes.
Cyclosporine
Increased Xanurin exposure; limit Xanuric dose.
Niacin (lipid-lowering doses)
Increased risk of muscle-related adverse events. Use with caution.
Gemfibrozil and other fibrates
Increased risk of myopathy/rhabdomyolysis when co-administered with Xanuric. Monitor closely for muscle symptoms and CK levels.
Strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin, HIV protease inhibitors)
May significantly increase plasma concentrations of Xanurin, increasing risk of myopathy/rhabdomyolysis. Avoid co-administration or use lower Xanuric dose with careful monitoring.
Storage
Store below 30°C (86°F) in a dry place, away from light and moisture. Keep out of reach of children. Do not freeze.
Overdose
No specific antidote for Xanuric overdose. In case of overdose, initiate symptomatic and supportive measures. Patients should be monitored for liver function tests (LFTs) and creatine kinase (CK) levels. Hemodialysis is unlikely to be of benefit due to extensive protein binding.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation due to potential harm to the fetus/infant. Xanurin may cause fetal harm when administered to a pregnant woman. Women of childbearing potential should use effective contraception.

WHO Essential Medicine
YesClinical Trials
Xanuric has undergone extensive clinical trials, including the XANURIC-CV Outcome Trial, demonstrating its efficacy in reducing LDL-C by 30-50% and significantly lowering the risk of major adverse cardiovascular events in both primary and secondary prevention settings.
Lab Monitoring
- Liver function tests (ALT, AST) before treatment, at 12 weeks after initiation or dose elevation, and periodically thereafter (e.g., annually).
- Lipid profile (total cholesterol, LDL-C, HDL-C, triglycerides) to assess efficacy, typically 4-12 weeks after initiation or dose adjustment.
- Creatine kinase (CK) levels if muscle symptoms (pain, tenderness, weakness) occur, and prior to treatment in high-risk patients.
Doctor Notes
- Emphasize the importance of adherence to medication and concurrent lifestyle modifications (diet, exercise) for optimal lipid management.
- Educate patients on potential muscle-related symptoms (myalgia, weakness, tenderness) and instruct them to report these promptly.
- Regular monitoring of lipid profile and liver function tests (LFTs) is crucial for assessing efficacy and safety.
- Consider potential drug interactions, especially with strong CYP3A4 inhibitors, fibrates, and cyclosporine.
Patient Guidelines
- Take Xanuric exactly as prescribed by your doctor, usually once daily in the evening.
- Do not stop taking it without consulting your doctor, even if you feel well, as cholesterol levels may rise again.
- Report any unexplained muscle pain, tenderness, or weakness, especially if accompanied by fever or malaise, to your doctor immediately.
- Inform your doctor about all other medications, supplements, and herbal products you are taking.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it's almost time for the next dose (within 12 hours), skip the missed dose and continue with your regular schedule. Do not double the dose to make up for a missed one.
Driving Precautions
Xanuric is generally not expected to impair the ability to drive or operate machinery. However, if you experience dizziness, visual disturbances, or other neurological side effects, avoid such activities until you are sure you can perform them safely.
Lifestyle Advice
- Maintain a healthy diet low in saturated and trans fats, and rich in fruits, vegetables, and whole grains.
- Engage in regular physical activity (e.g., 30 minutes of moderate-intensity exercise most days of the week) as advised by your doctor.
- Avoid smoking and limit alcohol consumption, as these can negatively impact cholesterol levels and cardiovascular health.
- Achieve and maintain a healthy weight.
