Adults
Initially 40 mg once daily, may be increased to 80 mg once daily after 2 weeks if serum uric acid is > 6 mg/dL.
Elderly
No dosage adjustment required.
Renal_impairment
No dosage adjustment required for mild to moderate impairment. Use with caution in severe renal impairment.
How to Take
Oral, with or without food.
Mechanism of Action
Febuxostat selectively inhibits xanthine oxidase, an enzyme responsible for uric acid production, thereby reducing serum uric acid levels.
Pharmacokinetics
Onset
Uric acid lowering effect within 1-2 weeks.
Excretion
Approximately 49% excreted renally and 45% fecally.
Half life
Approximately 5-9 hours.
Absorption
Rapidly and extensively absorbed (approx 84%), peak plasma concentrations within 1-1.5 hours.
Metabolism
Extensively metabolized in the liver via conjugation and oxidation.
Side Effects
Contraindications
- Hypersensitivity to febuxostat or any component.
- Concurrent treatment with azathioprine or mercaptopurine.
Drug Interactions
Theophylline
Febuxostat may increase theophylline levels, monitor closely.
Azathioprine/Mercaptopurine
Febuxostat increases their plasma concentrations, leading to severe myelosuppression. Concurrent use is contraindicated.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Management involves symptomatic and supportive care. No specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether febuxostat is excreted in human milk.

Clinical Trials
Extensive clinical trials (e.g., APEX, FACT, CONFIRMS) have demonstrated the efficacy and safety of febuxostat in reducing serum uric acid levels and preventing gout flares in patients with hyperuricemia and gout.
Lab Monitoring
- Serum uric acid levels (regularly until target achieved).
- Liver function tests (baseline and periodically).
Doctor Notes
- Initiate treatment with colchicine or NSAIDs to prevent gout flares during the initial phase of febuxostat therapy, as uric acid mobilization can trigger acute attacks.
- Monitor liver function tests (LFTs) at baseline and periodically during treatment due to potential hepatotoxicity.
- Regularly assess serum uric acid levels to ensure the therapeutic target is achieved and maintained.
Patient Guidelines
- Take as directed by your doctor.
- Do not stop taking without consulting your doctor, even if gout symptoms improve.
- Drink plenty of fluids to help prevent kidney stones.
Missed Dose Advice
Take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
Driving Precautions
May cause dizziness or somnolence. Patients should be cautioned about operating hazardous machinery, including automobiles, until they are reasonably certain that febuxostat therapy does not adversely affect their ability to perform such activities.
Lifestyle Advice
- Maintain a healthy diet, avoiding foods high in purines.
- Increase fluid intake.
- Regular exercise.
