Overview of the medicine
Xbac 500 mg Tablet is a broad-spectrum antibiotic used to treat a variety of bacterial infections, including those of the respiratory tract, urinary tract, skin and soft tissue, and ear, nose, and throat. It belongs to the cephalosporin class of antibiotics.
Uses & Indications
Dosage
Adults
For most infections, 250 mg to 500 mg twice daily. Duration of treatment varies depending on the infection severity and type.
Elderly
Similar to adult dosage, but caution and dosage adjustment may be necessary in elderly patients with impaired renal function.
Renal_impairment
Dosage should be adjusted based on creatinine clearance (CrCl). For CrCl 10-29 mL/min, 250 mg once daily. For CrCl <10 mL/min, 250 mg every 48 hours.
How to Take
Take Xbac 500 mg Tablet orally, preferably with or after food, to enhance absorption and reduce potential gastrointestinal upset. Swallow the tablet whole; do not crush, chew, or divide.
Mechanism of Action
Cefuroxime Axetil, after oral administration, is hydrolyzed to cefuroxime. Cefuroxime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Typically, therapeutic concentrations are reached within 1-2 hours after oral dosing.
Excretion
Primarily excreted unchanged by the kidneys, via glomerular filtration and tubular secretion.
Half life
Plasma half-life is approximately 1.2 to 1.5 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract after oral administration and rapidly hydrolyzed by esterase enzymes in the intestinal mucosa and blood to cefuroxime. Absorption is enhanced when taken with food.
Metabolism
Cefuroxime is not significantly metabolized in the liver.
Side Effects
Contraindications
- •Known hypersensitivity to cefuroxime, other cephalosporins, or to any component of Xbac 500 mg Tablet.
- •History of severe hypersensitivity reaction to any other type of beta-lactam antibacterial agent (e.g., penicillins, carbapenems).
Drug Interactions
Probenecid
Concomitant use with probenecid increases the area under the serum concentration versus time curve (AUC) of cefuroxime and prolongs its half-life.
Antacids and H2-receptor antagonists
May decrease the bioavailability of cefuroxime. It is recommended to administer Xbac at least one hour before or two hours after antacids.
Oral anticoagulants (e.g., Warfarin)
May cause an increase in INR (International Normalized Ratio). Close monitoring of INR is advised when Xbac is co-administered with oral anticoagulants.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose typically include gastrointestinal disturbances such as nausea, vomiting, and diarrhea. In rare cases, neurological sequelae including convulsions may occur. Management is symptomatic and supportive. Hemodialysis and peritoneal dialysis can effectively remove cefuroxime.
Pregnancy & Lactation
Pregnancy Category B: Studies in animals have shown no evidence of impaired fertility or harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Lactation: Cefuroxime is excreted in human milk in small quantities. Caution should be exercised when Xbac is administered to a nursing mother.
