Overview of the medicine
Xbac 750 mg Tablet contains Levofloxacin, a fluoroquinolone antibiotic used to treat a wide range of bacterial infections including respiratory tract infections, skin and soft tissue infections, urinary tract infections, and more.
Uses & Indications
Dosage
Adults
750 mg once daily for 5-14 days depending on the infection type and severity.
Elderly
No dosage adjustment necessary based solely on age, but consider renal function.
Renal_impairment
Dosage adjustment required for creatinine clearance <50 mL/min. For CrCl 20-49 mL/min, initial dose 750 mg, then 500 mg every 48 hours. For CrCl 10-19 mL/min, initial dose 750 mg, then 250 mg every 48 hours.
How to Take
Take orally with or without food. Ensure adequate hydration. Do not take antacids containing magnesium or aluminum, sucralfate, iron supplements, or multivitamins containing zinc within 2 hours before or after taking Xbac.
Mechanism of Action
Levofloxacin exerts its bactericidal effect by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination.
Pharmacokinetics
Onset
Within 1-2 hours for therapeutic effect.
Excretion
Mainly excreted unchanged in the urine, with a small amount excreted in the feces.
Half life
Approximately 6 to 8 hours.
Absorption
Rapid and complete absorption after oral administration; peak plasma concentrations reached within 1-2 hours.
Metabolism
Minimally metabolized in the liver; primarily excreted as unchanged drug.
Side Effects
Contraindications
- •Hypersensitivity to levofloxacin, other quinolones, or any excipients of the product.
- •History of tendinitis or tendon rupture associated with fluoroquinolone use.
- •Epilepsy.
- •Children or adolescents (under 18 years of age).
Drug Interactions
Warfarin
May enhance the anticoagulant effects; monitor INR/PT closely.
Corticosteroids
Increased risk of tendinopathy and tendon rupture, especially in elderly patients.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May increase the risk of CNS stimulation and convulsive seizures when co-administered with quinolones.
Antacids, Sucralfate, Metal Cations (e.g., iron, zinc)
Reduce oral absorption of levofloxacin; administer Xbac at least 2 hours before or 2 hours after these agents.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be considered. Symptomatic and supportive treatment should be initiated. Monitor ECG due to potential for QT prolongation. Hemodialysis or peritoneal dialysis is not effective in removing levofloxacin from the body.
Pregnancy & Lactation
Pregnancy Category C. Not recommended during pregnancy or breastfeeding unless the potential benefits outweigh the risks. Animal studies show potential for arthropathy in offspring. Levofloxacin is excreted in breast milk.
