Overview of the medicine
Xerocid (Cefixime) is a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including respiratory tract infections, urinary tract infections, otitis media, and pharyngitis.
Uses & Indications
Dosage
Adults
Adults and children over 10 years or weighing over 50 kg: 400 mg once daily or 200 mg twice daily. For uncomplicated gonorrhea, a single dose of 400 mg.
Elderly
No specific dose adjustment necessary unless renal function is significantly impaired.
Renal_impairment
CrCl 21-60 mL/min: 300 mg daily. CrCl <20 mL/min or hemodialysis: 200 mg daily. Peritoneal dialysis: 200 mg daily.
How to Take
Can be taken with or without food. To minimize gastrointestinal upset, it is often recommended to take with food.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which inhibits the third and last stage of bacterial cell wall synthesis. Cell lysis is then mediated by bacterial cell wall autolytic enzymes such as autolysins; it is possible that cefixime interferes with an autolysin inhibitor.

Onset
Variable, generally within hours for antibacterial effect.
Excretion
Primarily renal (glomerular filtration and tubular secretion) with some biliary excretion.

Absorption
Approximately 40-50% orally absorbed. Food slightly delays Tmax but does not significantly alter Cmax or AUC.
Metabolism
Limited hepatic metabolism; primarily excreted unchanged.
