Description
Overview of the medicine
Ximeclav is an antibiotic combination containing Cefixime, a third-generation cephalosporin, and Clavulanic Acid, a beta-lactamase inhibitor. It is used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Cefixime 200 mg + Clavulanic Acid 125 mg, twice daily for 5-10 days, depending on the severity of infection.
Elderly
No specific dose adjustment is required for elderly patients unless there is significant renal impairment.
Renal_impairment
Dosage adjustment is necessary in patients with severe renal impairment (creatinine clearance < 30 mL/min).
How to Take
To be taken orally, with or without food. For best absorption and to reduce gastrointestinal upset, it is advisable to take with food.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. Clavulanic acid is a beta-lactamase inhibitor that protects Cefixime from degradation by beta-lactamase enzymes produced by resistant bacteria, thereby extending its antibacterial spectrum.
Pharmacokinetics
Onset
Rapid absorption with peak plasma concentrations occurring within 2-6 hours for Cefixime and 1-2 hours for Clavulanic Acid.
Excretion
Cefixime is primarily excreted renally via glomerular filtration and tubular secretion. Clavulanic acid is excreted renally and non-renally.
Half life
Cefixime: approximately 3-4 hours. Clavulanic Acid: approximately 1 hour.
Absorption
Cefixime is 40-50% absorbed orally. Clavulanic acid is well absorbed orally.
Metabolism
Cefixime undergoes limited hepatic metabolism. Clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- Hypersensitivity to Cefixime or other cephalosporins
- Hypersensitivity to Clavulanic Acid or other beta-lactam antibiotics
- History of cholestatic jaundice or hepatic dysfunction associated with penicillin or cephalosporin use
Drug Interactions
Warfarin
May increase anticoagulant effect and risk of bleeding; monitor INR closely.
Probenecid
May increase Cefixime plasma concentrations and prolong its half-life.
Oral Contraceptives
May decrease the efficacy of oral contraceptives; advise additional contraceptive methods.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children. Reconstituted dry syrup should be stored in a refrigerator (2-8°C) and used within 7-14 days.
Overdose
In case of overdose, gastric lavage may be considered. Symptomatic and supportive treatment is recommended. Hemodialysis is not likely to be beneficial.
