Description
Overview of the medicine
Xirocip 200 mg Injection contains Ciprofloxacin, a fluoroquinolone antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
200-400 mg intravenously every 8-12 hours, depending on the severity and type of infection. Infuse slowly over 60 minutes.
Elderly
Adjust dosage based on renal function; generally similar to adults.
Renal_impairment
Dosage adjustment is necessary for patients with creatinine clearance < 30 mL/min (e.g., 200 mg every 18-24 hours).
How to Take
Administer intravenously by slow infusion over 60 minutes. Do not inject rapidly.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
Pharmacokinetics
Onset
Within 30-60 minutes after intravenous administration.
Excretion
Primarily renal (about 50-70% as unchanged drug) and to a lesser extent through feces.
Half life
Approximately 3-5 hours.
Absorption
Rapid and extensive absorption after intravenous administration, 100% bioavailability.
Metabolism
Partially metabolized in the liver to less active metabolites.
Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or other quinolones
- Concomitant administration with tizanidine
- History of tendinitis or tendon rupture associated with fluoroquinolone use
Drug Interactions
Warfarin
Enhanced anticoagulant effect, requiring close INR monitoring.
Tizanidine
Concomitant use is contraindicated due to increased tizanidine plasma concentrations and enhanced hypotensive and sedative effects.
Theophylline
Increased risk of theophylline toxicity due to reduced clearance.
Corticosteroids
Increased risk of severe tendon disorders.
Storage
Store below 30°C (86°F), protected from light. Do not freeze. Keep out of reach of children.
Overdose
Symptoms may include renal toxicity, reversible renal dysfunction, and sometimes central nervous system effects. Treatment is supportive; hemodialysis may be beneficial in removing ciprofloxacin from the body.
Pregnancy & Lactation
Use in pregnancy only if the potential benefit outweighs the potential risk to the fetus. Ciprofloxacin is excreted into breast milk; therefore, caution is advised or consider alternatives. Consult a doctor.

Lab Monitoring
- Renal function tests (creatinine, BUN)
- Liver function tests (ALT, AST)
- Complete blood count (CBC)
- ECG (in patients at risk for QT prolongation or electrolyte imbalance)
Doctor Notes
- Carefully assess patient history for hypersensitivity to quinolones and risk factors for tendinopathy or CNS effects.
- Monitor renal function in elderly and renally impaired patients, adjusting dosage as needed.
- Educate patients on potential severe side effects (e.g., tendon pain, numbness) and to seek immediate medical attention if they occur.
- Avoid concomitant use with tizanidine due to contraindication.
Patient Guidelines
- Complete the full course of treatment as prescribed by your doctor, even if symptoms improve.
- Report any severe or persistent side effects to your doctor immediately.
- Do not take this medicine with tizanidine.
- Stay well-hydrated during treatment.
Missed Dose Advice
If a dose is missed, take it as soon as remembered. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.
Driving Precautions
May cause dizziness, lightheadedness, or visual disturbances, which can impair your ability to drive or operate machinery. Patients should be advised to exercise caution until they know how the medicine affects them.
