Description
Overview of the medicine
Xofena 120 mg Tablet contains Fexofenadine, a non-drowsy antihistamine used to relieve symptoms of seasonal allergic rhinitis and chronic idiopathic urticaria.
Uses & Indications
Dosage
Adults
For seasonal allergic rhinitis and chronic idiopathic urticaria: 120 mg once daily or 60 mg twice daily. The dose may vary based on clinical response and physician's advice.
Elderly
No specific dose adjustment is generally required in elderly patients, but caution is advised in those with reduced renal function.
Renal_impairment
For patients with impaired renal function, a starting dose of 60 mg once daily is recommended.
How to Take
Take orally with water, preferably before a meal. Do not take with fruit juices (e.g., grapefruit, orange, apple) as they may reduce the bioavailability of fexofenadine.
Mechanism of Action
Fexofenadine selectively blocks peripheral H1-histamine receptors, preventing histamine from binding and thereby inhibiting the effects of histamine in allergic reactions without significant central nervous system effects.
Pharmacokinetics
Onset
Onset of action typically occurs within 1 hour.
Excretion
Mainly excreted unchanged in feces (approximately 80%) and urine (approximately 12%).
Half life
Approximately 11-15 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Peak plasma concentrations are reached approximately 1-3 hours after oral administration.
Metabolism
Minimally metabolized (approximately 5%) in the liver. Primarily transported via P-glycoprotein.
Side Effects
Contraindications
- Hypersensitivity to fexofenadine or any component of the formulation.
Drug Interactions
Erythromycin and Ketoconazole
Can increase plasma levels of fexofenadine, though clinical significance is generally minimal.
Antacids (aluminum and magnesium-containing)
May reduce the absorption and bioavailability of fexofenadine. Separate administration by at least 2 hours.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive measures should be initiated. Hemodialysis does not effectively remove fexofenadine from the blood.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Fexofenadine is excreted in human breast milk; use with caution in nursing mothers.
