Description
Overview of the medicine
Xofena 30 mg Suspension is a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. Its active ingredient is Cefixime. It works by preventing the growth of bacteria.
Uses & Indications
Dosage
Adults
For children, the recommended dose is 8 mg/kg/day administered as a single dose or in two divided doses. For a 30 mg/5 ml suspension, this typically translates to 1.25 ml to 2.5 ml twice daily for children weighing 6-12 kg depending on the infection. Consult a physician for specific dosing.
Elderly
Dosage adjustments are usually not necessary unless there is severe renal impairment.
Renal_impairment
In patients with creatinine clearance < 30 mL/min, the dose should be reduced to 4 mg/kg/day.
How to Take
Shake the bottle well before each use. Administer orally with the provided measuring spoon or dropper. It can be taken with or without food.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the final transpeptidation step of peptidoglycan synthesis, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Generally within 1-2 hours for therapeutic effect.
Excretion
Primarily excreted unchanged in the urine (about 50%) and bile (about 10%).
Half life
Plasma half-life is typically 3-4 hours, which allows for once or twice-daily dosing.
Absorption
Approximately 40-50% orally absorbed. Peak plasma concentrations are achieved 2-6 hours after administration. Food does not significantly alter absorption.
Metabolism
Cefixime is not extensively metabolized; a small portion may undergo metabolism.
Side Effects
Contraindications
- •Hypersensitivity to Cefixime or other cephalosporin antibiotics.
- •Hypersensitivity to penicillin (due to potential cross-reactivity).
Drug Interactions
Nifedipine
May increase Cefixime levels.
Carbamazepine
May increase carbamazepine levels.
Warfarin and other anticoagulants
May increase prothrombin time (PT) or INR, increasing bleeding risk.
Storage
Store the dry powder below 30°C, protected from light and moisture. Once reconstituted, store the suspension in a refrigerator (2-8°C) and discard after 7-14 days (check specific product leaflet). Do not freeze.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed during pregnancy. Cefixime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
