Description
Overview of the medicine
Zacort (Deflazacort) is a corticosteroid used to treat a wide range of inflammatory and autoimmune conditions, including Duchenne Muscular Dystrophy, rheumatic disorders, allergic states, and certain dermatologic, respiratory, hematologic, neoplastic, gastrointestinal, and edematous conditions. It possesses anti-inflammatory and immunosuppressive properties.
Uses & Indications
Dosage
Adults
Dosage varies widely based on the condition being treated, severity, and patient response. Typically initiated at 6-90 mg/day, gradually tapered. For DMD, typically 0.9 mg/kg/day.
Elderly
No specific dose adjustment for elderly patients, but use with caution due to increased risk of side effects.
Renal_impairment
No specific dose adjustment required for renal impairment, but caution is advised.
How to Take
Take orally, with or without food. If gastric irritation occurs, take with food or milk. Follow physician's instructions carefully. Do not stop abruptly.
Mechanism of Action
Deflazacort is a synthetic glucocorticoid that acts on glucocorticoid receptors found in most tissues. It inhibits the release of various inflammatory mediators and suppresses immune responses by altering gene expression, thus reducing inflammation and immunosuppression.
Pharmacokinetics
Onset
Onset of action varies depending on the condition being treated, usually within hours to days.
Excretion
Excreted predominantly in urine (70%) and feces (30%) mainly as metabolites.
Half life
The active metabolite, 21-desacetyl deflazacort, has a plasma half-life of approximately 1.5-2 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract and converted to the active metabolite 21-desacetyl deflazacort.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4, to its active metabolite 21-desacetyl deflazacort.
Side Effects
Contraindications
- Systemic fungal infections
- Hypersensitivity to deflazacort or any components
Drug Interactions
NSAIDs
Increased risk of gastrointestinal ulceration.
Antacids
May reduce absorption; separate administration by at least 2 hours.
Warfarin
May alter anticoagulant effects; monitor INR.
Live vaccines
Avoid during corticosteroid therapy due to immunosuppression.
CYP3A4 Inducers (e.g., rifampin, phenytoin)
May decrease deflazacort levels; dose adjustment may be needed.
CYP3A4 Inhibitors (e.g., ketoconazole, clarithromycin)
May increase deflazacort levels; monitor for increased side effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is rare. Symptoms may include exaggeration of known side effects. Treatment is symptomatic and supportive; no specific antidote. Gradual tapering of dose is important.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Small amounts may pass into breast milk; use with caution or consider discontinuing breastfeeding.

- Blood glucose levels
- Blood pressure
- Bone mineral density (with long-term use)
- Electrolytes
- Eye examinations (for cataracts/glaucoma)
Doctor Notes
- Emphasize gradual tapering of dose to prevent adrenal crisis.
- Monitor patients for signs of infection, blood glucose, bone density, and eye health during long-term therapy.
- Counsel patients on potential behavioral changes and mood disturbances.
Patient Guidelines
- Do not stop taking Zacort abruptly, as it can lead to withdrawal symptoms or adrenal insufficiency. Consult your doctor for gradual tapering.
- Report any unusual side effects, especially signs of infection, severe mood changes, or stomach upset.
- Carry a steroid warning card, especially if you are on long-term therapy.
