Description
Overview of the medicine
Zosert (Sertraline) is a selective serotonin reuptake inhibitor (SSRI) antidepressant used to treat depression, obsessive-compulsive disorder (OCD), panic disorder, post-traumatic stress disorder (PTSD), premenstrual dysphoric disorder (PMDD), and social anxiety disorder.
Uses & Indications
Dosage
Adults
Initial dose 25-50 mg once daily; may be increased gradually up to a maximum of 200 mg once daily, depending on indication and patient response.
Elderly
Lower starting doses and slower titration may be considered; maximum dose of 200 mg/day.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Administer orally once daily, in the morning or evening. Can be taken with or without food.
Mechanism of Action
Sertraline selectively inhibits the reuptake of serotonin (5-HT) by the presynaptic neuron in the central nervous system, thereby increasing the concentration of serotonin in the synaptic cleft.
Pharmacokinetics
Onset
Initial effects may be observed within 1-2 weeks, but full therapeutic effect usually takes 2-4 weeks.
Excretion
Excreted primarily in feces (40-45%) and urine (13-16%) as metabolites.
Half life
Approximately 26 hours (range 22-36 hours).
Absorption
Well absorbed from the gastrointestinal tract; peak plasma concentrations reached in 4.5-8.4 hours. Food enhances absorption.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19, to N-desmethylsertraline, an active but less potent metabolite.
Side Effects
Contraindications
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI treatment
- Hypersensitivity to sertraline or any component of the formulation
- Concomitant use with pimozide
Drug Interactions
MAOIs
Increased risk of serotonin syndrome, may be fatal.
Pimozide
Increased pimozide levels, QT prolongation.
Warfarin
May increase prothrombin time and bleeding risk.
Other serotonergic drugs (e.g., other SSRIs, SNRIs, triptans, fentanyl, tramadol)
Increased risk of serotonin syndrome.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include somnolence, vomiting, tachycardia, tremor, agitation, dizziness, and rarely, seizures and coma. Treatment is supportive and symptomatic; ensure adequate airway, oxygenation, and ventilation. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Small amounts are excreted in breast milk; consult a doctor before breastfeeding.
