Description
Overview of the medicine
Ampicillin is a beta-lactam antibiotic used to treat a wide range of bacterial infections. It is a broad-spectrum penicillin that works by inhibiting bacterial cell wall synthesis. The 250 mg injection formulation is typically used for parenteral administration in hospital or clinic settings.
Uses & Indications
Dosage
Adults
Typical dose: 250 mg to 1 g every 6 hours, given IM or IV. Higher doses (up to 12 g/day) may be used for severe infections.
Elderly
Dosage may need to be adjusted based on renal function.
Renal_impairment
Dosage interval should be extended or dose reduced in patients with significant renal impairment. Consult specific guidelines based on creatinine clearance.
How to Take
Acmecilin 250 mg Injection can be administered either intramuscularly (IM) or intravenously (IV). For IV administration, the powder should be reconstituted with sterile water for injection and then diluted further before slow intravenous infusion. For IM administration, reconstitute and inject deep into a large muscle.
Mechanism of Action
Ampicillin exerts its bactericidal action by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan polymers, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Within minutes for IV, 1-2 hours for IM.
Excretion
Primarily excreted unchanged in the urine via glomerular filtration and tubular secretion. A small amount is excreted in bile.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Rapidly absorbed after intramuscular (IM) injection. Peak plasma concentrations are reached within 1-2 hours. Intravenous (IV) administration provides immediate and complete bioavailability.
Metabolism
Limited metabolism; a small portion is hydrolyzed to penicilloic acid.
Side Effects
Contraindications
- •Hypersensitivity to ampicillin, any other penicillin, or any component of the formulation.
- •History of severe allergic reaction to any beta-lactam antibiotic (e.g., anaphylaxis).
- •Infectious mononucleosis (may cause a non-allergic rash).
Drug Interactions
Probenecid
Increases and prolongs plasma levels of ampicillin by decreasing renal tubular secretion.
Allopurinol
Increased incidence of rash when co-administered with ampicillin.
Tetracyclines
May antagonize the bactericidal effect of ampicillin.
Aminoglycosides
Physical inactivation can occur if mixed in the same IV solution; administer separately.
Oral Contraceptives
May reduce the efficacy of oral contraceptives.
Storage
Store below 30°C in a dry place, away from light. Reconstituted solutions should be used immediately or stored as per specific manufacturer instructions, typically for a short period under refrigeration.
Overdose
Symptoms of overdose may include severe gastrointestinal disturbances (nausea, vomiting, diarrhea), neurological symptoms (convulsions, especially in patients with impaired renal function or high doses). Treatment is supportive and symptomatic; hemodialysis may be effective in removing ampicillin from the bloodstream.
Pregnancy & Lactation
Pregnancy Category B. Ampicillin is generally considered safe during pregnancy when clearly needed. It is excreted in small amounts into breast milk; caution should be exercised when administered to a nursing mother. Consult a healthcare professional.
