Acmecilin 500 mg Injection is a broad-spectrum antibiotic belonging to the third-generation cephalosporin class, used to treat a wide range of bacterial infections. It is administered parenterally.
Uses & Indications
Dosage
Adults
Typical adult dosage is 0.5-2g every 12-24 hours, depending on the severity of infection. For 500 mg, it might be 500 mg every 8-12 hours for less severe infections or as a maintenance dose after initial higher doses.
Elderly
Similar to adult dosage, but caution with renal function. Dose adjustment may be necessary based on creatinine clearance.
Renal_impairment
Dose adjustment required based on creatinine clearance. For CrCl <10 mL/min, reduce dose by 50%. For CrCl 10-50 mL/min, administer usual dose less frequently or reduce dose.
How to Take
Acmecilin 500 mg injection is reconstituted with sterile water for injection or an appropriate diluent and administered via slow intravenous (IV) infusion over 30 minutes or deep intramuscular (IM) injection.
Mechanism of Action
Acmecilin exerts its bactericidal action by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, preventing the final transpeptidation step of peptidoglycan synthesis, thereby inhibiting cell wall biosynthesis. This leads to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, within minutes for IV administration, 30-60 minutes for IM administration.
Excretion
Primarily renal excretion (glomerular filtration and tubular secretion). Some biliary excretion also occurs.
Half life
Approximately 6-9 hours.
Absorption
Rapid and complete absorption after intramuscular (IM) or intravenous (IV) administration. Peak plasma concentrations are reached within 2-3 hours after IM injection.
Metabolism
Minimal hepatic metabolism; primarily excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to cephalosporins or any other beta-lactam antibiotics (e.g., penicillins)
- •Infants with hyperbilirubinemia (especially premature infants) due to risk of bilirubin encephalopathy
Drug Interactions
Probenecid
May increase and prolong plasma concentrations of Acmecilin.
Aminoglycosides
Increased risk of nephrotoxicity.
Oral Anticoagulants
May increase the anticoagulant effect. Monitor INR/PT.
Loop Diuretics (e.g., Furosemide)
Increased risk of nephrotoxicity.
Storage
Store below 30°C in a dry place, away from light. Reconstituted solution should be used immediately or stored for a short period as directed by manufacturer.
Overdose
In case of overdose, symptomatic and supportive measures should be taken. Hemodialysis or peritoneal dialysis may be useful in enhancing elimination, especially in patients with impaired renal function.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excreted in breast milk in small amounts; use with caution during lactation. Consult a physician.
