Description
Overview of the medicine
Amoxicillin and clavulanic acid is a combination antibiotic used to treat a wide range of bacterial infections. Amoxicillin is a penicillin-type antibiotic that fights bacteria, while clavulanic acid helps prevent certain bacteria from becoming resistant to amoxicillin. This suspension is particularly suited for pediatric use.
Uses & Indications
Dosage
Adults
Not typically used in 125mg suspension for adults; higher strengths are usually prescribed.
Elderly
Dosage as for adults, but consider renal function and adjust if necessary.
Pediatric
20-45 mg/kg/day (based on amoxicillin component) in divided doses every 8 or 12 hours, depending on severity and infection. For 1-6 years, typically 5-10 ml (1-2 teaspoonfuls) 2-3 times daily.
Renal_impairment
Dosage adjustment required based on creatinine clearance.
How to Take
Administer orally, preferably at the start of a meal to minimize potential gastrointestinal intolerance. Shake the bottle well before each use and use the provided measuring device.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). Clavulanic acid is a beta-lactamase inhibitor that irreversibly binds to and inactivates beta-lactamase enzymes produced by bacteria, thus protecting amoxicillin from degradation.
Pharmacokinetics
Onset
Rapid, typically within 1 hour.
Excretion
Primarily renal (glomerular filtration and tubular secretion).
Half life
Amoxicillin: approximately 1 hour; Clavulanic acid: approximately 1 hour.
Absorption
Both components are well absorbed orally; clavulanate is absorbed more rapidly. Food can affect absorption slightly but generally recommended with food to reduce GI upset.
Metabolism
Amoxicillin undergoes limited metabolism; clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- •History of hypersensitivity to penicillin or other beta-lactam antibiotics (e.g., cephalosporins)
- •History of amoxicillin/clavulanic acid-associated jaundice or hepatic dysfunction
- •Infectious mononucleosis
Drug Interactions
Probenecid
Increases amoxicillin blood levels by reducing renal tubular secretion.
Allopurinol
Increased risk of skin rash.
Methotrexate
Increased methotrexate toxicity due to reduced renal excretion.
Oral Contraceptives
May reduce efficacy of oral contraceptives; advise additional contraceptive measures.
Anticoagulants (e.g., Warfarin)
May prolong prothrombin time; monitor INR carefully.
Storage
Store the dry powder below 25°C in a dry place, away from direct sunlight. After reconstitution, store the suspension in a refrigerator (2-8°C) and discard any unused portion after 7-14 days.
Overdose
Overdose may lead to gastrointestinal symptoms (e.g., abdominal pain, vomiting, diarrhea) and fluid/electrolyte imbalance. Renal dysfunction, crystalluria, and convulsions can also occur. Treatment is symptomatic and supportive. Hemodialysis can remove amoxicillin/clavulanic acid from circulation.
Pregnancy & Lactation
Pregnancy Category B. Should be used during pregnancy only if clearly needed. Both amoxicillin and clavulanic acid are excreted into breast milk. Generally considered compatible with breastfeeding, but caution is advised. Monitor infant for potential adverse effects like diarrhea, candidiasis, or skin rash.
