Overview of the medicine
Amoclav 875 mg tablet is a combination antibiotic containing amoxicillin and clavulanic acid, used to treat a wide range of bacterial infections resistant to amoxicillin alone.
Uses & Indications
Dosage
Adults
One 875 mg/125 mg tablet twice daily (every 12 hours) for 7 to 10 days, depending on the severity and type of infection.
Elderly
No specific dosage adjustment is generally required unless there is severe renal impairment. Monitor renal function.
Renal_impairment
For creatinine clearance 10-30 mL/min, 875 mg/125 mg every 24 hours. For creatinine clearance less than 10 mL/min, 500 mg/125 mg every 24 hours. Hemodialysis patients should receive 500 mg/125 mg every 24 hours, with an additional dose during and at the end of dialysis.
How to Take
Take orally, preferably at the start of a meal to minimize potential gastrointestinal intolerance and enhance absorption. Swallow the tablet whole with water.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis, leading to cell lysis. Clavulanic acid inhibits beta-lactamase enzymes produced by some bacteria, which would otherwise inactivate amoxicillin, thus extending amoxicillin's spectrum of activity.

Onset
Peak plasma concentrations are achieved approximately 1-2 hours post-administration.
Excretion
Primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion.
Half life
Amoxicillin: approximately 1-1.3 hours; Clavulanic acid: approximately 1 hour.
Absorption
Both amoxicillin and clavulanic acid are well absorbed from the gastrointestinal tract after oral administration, with clavulanic acid enhancing the absorption of amoxicillin.
Metabolism
Amoxicillin is largely excreted unchanged; clavulanic acid is extensively metabolized to inactive compounds.
Side Effects
Contraindications
- •History of hypersensitivity to penicillin, cephalosporins, or any other beta-lactam antibiotics.
- •Previous history of cholestatic jaundice or hepatic dysfunction associated with amoxicillin/clavulanic acid therapy.
Drug Interactions
Probenecid
Concomitant use with probenecid may decrease the renal tubular secretion of amoxicillin, resulting in increased and prolonged blood levels of amoxicillin.
Allopurinol
Concomitant administration with allopurinol may increase the incidence of skin rashes, particularly in patients with hyperuricemia.
Methotrexate
Amoxicillin may reduce the renal excretion of methotrexate, potentially increasing methotrexate toxicity.
Oral Contraceptives
May reduce the efficacy of oral contraceptives; advise use of additional non-hormonal contraception.
Oral Anticoagulants (e.g., Warfarin)
May prolong prothrombin time. Close monitoring of coagulation parameters is recommended.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include gastrointestinal symptoms (nausea, vomiting, diarrhea) and disturbances of fluid and electrolyte balance. Renal impairment can lead to convulsions. Management is symptomatic and supportive. Gastric lavage may be performed, and hemodialysis can aid in the removal of the drug from circulation.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. However, use during pregnancy should be only if clearly needed. Both amoxicillin and clavulanic acid are excreted in small amounts into breast milk. Use with caution in nursing mothers, as it may cause sensitization or gastrointestinal upset in the infant.
