Description
Overview of the medicine
Amoxicillin is a penicillin-type antibiotic used to treat various bacterial infections, including those of the respiratory tract, urinary tract, skin, and ear.
Uses & Indications
Dosage
Adults
250-500 mg every 8 hours or 500-875 mg every 12 hours, depending on the severity and type of infection. Max 4.5 g/day.
Elderly
Similar to adults, but dose adjustment may be necessary in case of impaired renal function.
Renal_impairment
Dose reduction or extended dosing interval required based on creatinine clearance (e.g., 250-500 mg every 12-24 hours for severe impairment).
How to Take
Take orally with or without food. Swallow the capsule whole with water. Complete the full course of treatment even if symptoms improve.
Mechanism of Action
Amoxicillin works by inhibiting the synthesis of the bacterial cell wall, leading to bacterial cell death.
Pharmacokinetics
Onset
Approximately 30-60 minutes.
Excretion
Primarily renal (urine) as unchanged drug (50-70% of dose) within 6-8 hours.
Half life
About 1-1.5 hours in individuals with normal renal function.
Absorption
Rapidly and well absorbed after oral administration, with good bioavailability.
Metabolism
Limited; approximately 10-25% of the dose is metabolized, primarily by hydrolysis of the beta-lactam ring.
Side Effects
Contraindications
- Known hypersensitivity to amoxicillin, any other penicillin, or to any of the excipients.
- History of severe immediate hypersensitivity reaction (e.g., anaphylaxis) to another beta-lactam agent (e.g., cephalosporin, carbapenem or monobactam).
Drug Interactions
Warfarin
May prolong prothrombin time; monitor INR closely.
Allopurinol
Increased incidence of skin rashes, especially in patients with hyperuricemia.
Methotrexate
Increased methotrexate toxicity due to reduced renal clearance.
Oral Contraceptives
May reduce the efficacy of oral contraceptives (rarely).
Storage
Store in a cool, dry place below 30°C. Protect from moisture and direct light. Keep out of reach of children.
Overdose
Symptoms of overdose include gastrointestinal symptoms (nausea, vomiting, diarrhea) and disturbances in fluid and electrolyte balance. Management is supportive, including gastric lavage or induced emesis if ingestion is recent. Hemodialysis can remove amoxicillin from circulation.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation. Consult a doctor before use.
