Description
Overview of the medicine
Amoxicillin is a penicillin-type antibiotic used to treat a wide variety of bacterial infections. It works by stopping the growth of bacteria. This 500 mg injection is used for severe infections or when oral administration is not feasible.
Uses & Indications
Dosage
Adults
500 mg every 8 hours by IV or IM injection, or 1 g every 8 hours depending on severity. Max 6 g/day.
Elderly
Dosage adjustment may be necessary based on renal function.
Renal_impairment
Dosage adjustment required; e.g., CrCl 10-30 mL/min, 250-500 mg every 12 hours; CrCl <10 mL/min, 250-500 mg every 24 hours.
How to Take
Administer by slow intravenous (IV) injection over 3-5 minutes or by intravenous infusion. Can also be given by deep intramuscular (IM) injection. Reconstitute the powder for injection with sterile water for injection.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, typically within 30 minutes for IV.
Excretion
Primarily excreted unchanged in the urine via renal tubular secretion and glomerular filtration.
Half life
Approximately 1-1.5 hours (in adults with normal renal function).
Absorption
Rapidly and well absorbed after oral administration; for injection, bioavailability is 100%.
Metabolism
Minimally metabolized; about 10-25% is metabolized to inactive penicilloic acid.
Side Effects
Contraindications
- Hypersensitivity to amoxicillin or other penicillins
- History of severe allergic reaction to other beta-lactam antibiotics (e.g., cephalosporins)
Drug Interactions
Probenecid
Increases amoxicillin blood levels by reducing renal tubular secretion.
Allopurinol
Increased risk of rash.
Methotrexate
Increased toxicity of methotrexate due to reduced renal clearance.
Oral Contraceptives
May reduce efficacy of oral contraceptives.
Anticoagulants (e.g., Warfarin)
May prolong prothrombin time (INR).
Storage
Store powder for injection below 30°C in a dry place, protected from light. Do not freeze. Reconstituted solution should be used immediately or stored in a refrigerator for a limited time.
Overdose
Symptoms may include gastrointestinal disturbances (nausea, vomiting, diarrhea) and disturbances of fluid and electrolyte balances. Renal impairment may lead to crystalluria. Treatment is supportive, and hemodialysis can be used to remove amoxicillin from circulation.
Pregnancy & Lactation
Category B in pregnancy. Generally considered safe during pregnancy when clearly needed. Excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but monitor infant for potential side effects (e.g., diarrhea, candidiasis).

- Liver function tests (periodically during prolonged therapy)
- Complete blood count (in prolonged therapy)
Doctor Notes
- Always perform a thorough patient history regarding allergies to penicillins or cephalosporins before administration.
- Adjust dose in renal impairment.
- Monitor for signs of superinfection with prolonged use.
Patient Guidelines
- Complete the full course of treatment even if symptoms improve.
- Report any signs of allergic reaction immediately.
- Inform your doctor about all other medications you are taking.
Missed Dose Advice
If a dose is missed, administer it as soon as possible. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.
