Description
Overview of the medicine
Antif 125 mg Pediatric Drop is an antifungal medication containing Fluconazole, indicated for the treatment of various fungal infections in infants and children. It is formulated as an oral suspension for easy administration.
Uses & Indications
Dosage
Adults
Not typically administered as pediatric drops to adults; adult dosages vary by indication (e.g., 50-400 mg daily).
Elderly
Dosage may need adjustment based on renal function; refer to adult guidelines.
Children
Dosage is highly dependent on indication and weight. For Oropharyngeal candidiasis: 6 mg/kg on day 1, then 3 mg/kg once daily. For Systemic candidiasis: 6-12 mg/kg once daily.
Renal_impairment
Dosage reduction is necessary for patients with impaired renal function (creatinine clearance < 50 mL/min). Typically, an initial loading dose is given, followed by 50% of the usual daily dose.
How to Take
Administer orally using the provided calibrated dropper. The dose can be taken with or without food. Shake the bottle well before each use.
Mechanism of Action
Fluconazole inhibits the fungal cytochrome P450 enzyme 14α-demethylase, which is responsible for the demethylation of lanosterol to ergosterol. Ergosterol is a crucial component of the fungal cell membrane, and its impaired synthesis leads to increased cellular permeability, leakage of essential intracellular components, and eventual cell death.
Pharmacokinetics
Onset
Within 24-48 hours for symptomatic improvement.
Excretion
Primarily excreted unchanged in the urine (approximately 80% of the dose).
Half life
Approximately 20-30 hours (children may have shorter half-life).
Absorption
Well absorbed orally; bioavailability is >90% of IV dose. Peak plasma concentrations typically occur within 1-2 hours post-administration.
Metabolism
Minimally metabolized; approximately 11% of the drug is metabolized in the liver.
Side Effects
Contraindications
- •Hypersensitivity to Fluconazole or other azole antifungals.
- •Concomitant administration with terfenadine, astemizole, cisapride, pimozide, quinidine, or erythromycin.
Drug Interactions
Warfarin
Increased prothrombin time; monitor INR closely.
Phenytoin
Increased phenytoin levels; monitor levels and adjust dose.
Rifampicin
Decreased fluconazole levels; consider increasing fluconazole dose.
Cyclosporine, Tacrolimus
Increased levels of immunosuppressants; monitor levels.
Storage
Store below 30°C, protected from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose may include hallucinations and paranoid behavior. Treatment involves symptomatic and supportive measures, possibly gastric lavage, and hemodialysis may enhance elimination.
Pregnancy & Lactation
Generally not recommended during pregnancy, especially in the first trimester, unless the benefits outweigh the risks. Excreted into breast milk; caution advised during lactation.
