Description
Overview of the medicine
Arixon 500 mg Injection contains Ceftriaxone, a broad-spectrum, third-generation cephalosporin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Usual adult dose is 1-2 g intravenously (IV) or intramuscularly (IM) once daily, or in two equally divided doses. For severe infections, up to 4 g daily.
Elderly
No specific dosage adjustment needed for elderly patients with normal renal and hepatic function.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe impairment (creatinine clearance <10 mL/min), dosage should not exceed 2 g daily.
How to Take
For intramuscular injection, dissolve the powder in the accompanying diluent (e.g., Lidocaine 1%). For intravenous injection, dissolve in water for injection or compatible IV fluids and administer slowly over 30 minutes.
Mechanism of Action
Ceftriaxone inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Within 1-2 hours
Excretion
Approximately 50-60% excreted unchanged in urine and 40-50% in bile/feces.
Half life
6-9 hours
Absorption
Rapidly and completely absorbed after intramuscular injection. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Minimal hepatic metabolism; primarily by hydrolysis to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to ceftriaxone or other cephalosporins
- Neonates (especially premature) with hyperbilirubinemia
- Neonates receiving calcium-containing IV fluids
Drug Interactions
Loop diuretics
Increased nephrotoxicity (rare)
Aminoglycosides
Enhanced nephrotoxicity (rare)
Calcium-containing solutions
Formation of insoluble precipitates (contraindicated in neonates, caution in others)
Oral anticoagulants (e.g., Warfarin)
Increased anticoagulant effect (monitor INR)
Storage
Store below 30°C in a dry place, protected from light. Keep out of reach of children. Reconstituted solution should be used immediately or within a specified time.
Overdose
In case of overdose, symptomatic and supportive treatment is recommended. Hemodialysis or peritoneal dialysis is not effective in removing ceftriaxone.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy if clearly needed. Excreted in breast milk in small amounts; use with caution in nursing mothers.

Doctor Notes
- Consider culture and sensitivity tests before initiating treatment, especially for severe infections.
- Adjust dose in severe renal or hepatic impairment, though typically not required for mild-moderate cases.
- Monitor for signs of superinfection, particularly C. difficile-associated diarrhea.
- Avoid co-administration with calcium-containing solutions, especially in neonates.

- Complete the full course of treatment, even if symptoms improve.
- Report any severe or persistent side effects to your doctor immediately.
- Inform your doctor about all other medications you are taking, including over-the-counter drugs and herbal supplements.
