Description
Overview of the medicine
Asibalin (Gabapentin) is an anticonvulsant and neuropathic pain medication. It is used to treat partial seizures in epilepsy and neuropathic pain conditions like postherpetic neuralgia.
Uses & Indications
Dosage
Adults
Epilepsy: Initially 300 mg orally 3 times daily; may be increased to 900-1800 mg/day. Neuropathic pain: Initially 300 mg orally on Day 1, 300 mg twice daily on Day 2, and 300 mg three times daily on Day 3; then dose may be adjusted based on response up to 1800-3600 mg/day.
Elderly
Dosage should be adjusted based on renal function.
Renal_impairment
Dosage must be adjusted according to creatinine clearance. For CrCl < 30 mL/min, dosage should be significantly reduced.
How to Take
Asibalin can be taken with or without food. Swallow capsules whole with water. Do not crush or chew tablets (if extended-release).
Mechanism of Action
The exact mechanism of action of gabapentin is not fully understood, but it is believed to involve binding to the α2δ subunit of voltage-gated calcium channels, which reduces calcium influx and subsequent neurotransmitter release.
Pharmacokinetics
Onset
Varies; seizure control may take days to weeks, pain relief often within days.
Excretion
Excreted unchanged in urine. Renal impairment significantly prolongs half-life.
Half life
5 to 7 hours
Absorption
Well absorbed orally, with bioavailability decreasing at higher doses. Peak plasma concentrations are reached in 2-3 hours.
Metabolism
Not significantly metabolized in humans.
Side Effects
Contraindications
- Hypersensitivity to gabapentin or any component of the formulation.
- Acute pancreatitis due to gabapentin.
Drug Interactions
Opioids
May increase gabapentin concentrations and central nervous system depression. Dose reduction of opioids may be necessary.
Antacids
Aluminum- and magnesium-containing antacids may decrease gabapentin bioavailability. Take gabapentin at least 2 hours after antacid administration.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, dizziness, ataxia, lethargy, and mild diarrhea. Management is primarily supportive; hemodialysis can remove gabapentin.
Pregnancy & Lactation
Pregnancy Category C: Use only if the potential benefit outweighs the potential risk to the fetus. Gabapentin is excreted in human milk; use with caution in nursing mothers.

Lab Monitoring
- Monitor renal function (especially in elderly and those with kidney impairment).
- Observe for signs of behavioral changes or suicidal ideation.
Doctor Notes
- Counsel patients on the risk of suicidal ideation and CNS depression. Emphasize gradual withdrawal to prevent rebound seizures. Monitor renal function and adjust dose accordingly.
- Be aware of potential for misuse, especially in patients with a history of substance abuse.
Patient Guidelines
- Do not stop taking Asibalin suddenly, as this may increase the risk of seizures. Your doctor will gradually reduce your dose.
- Report any unusual mood changes, depression, or suicidal thoughts to your doctor immediately.
- Avoid driving or operating heavy machinery until you know how Asibalin affects you, as it may cause dizziness and drowsiness.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Asibalin may cause dizziness, drowsiness, and blurred vision. Patients should be warned about driving or operating machinery until they are certain the drug does not adversely affect their ability to perform such tasks.
Lifestyle Advice
- Maintain a balanced diet and regular exercise.
- Avoid alcohol consumption as it can worsen side effects.
