Description
Overview of the medicine
Azithromycin is a macrolide antibiotic used to treat a wide range of bacterial infections including respiratory tract infections, skin infections, ear infections, and sexually transmitted diseases.
Uses & Indications
Dosage
Adults
500 mg once daily for 3 days or 500 mg on day 1 then 250 mg daily for 4 days. For uncomplicated Chlamydia, a single 1g dose.
Elderly
Same as adults, no specific adjustment needed unless severe renal/hepatic impairment.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. Use with caution in severe impairment.
How to Take
Oral tablets/capsules can be taken with or without food. Oral suspension should be taken on an empty stomach (1 hour before or 2 hours after meals).
Mechanism of Action
Azithromycin works by inhibiting bacterial protein synthesis. It binds to the 50S ribosomal subunit of susceptible microorganisms, thereby preventing the translation of mRNA.

Variable, generally within 2-3 hours for peak plasma levels.
Excretion
Primarily excreted unchanged in bile.
Half life
Terminal half-life 2-4 days.
Absorption
Rapidly absorbed, bioavailability approximately 37%. Food affects absorption, particularly with oral suspension.

Side Effects
Contraindications

- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use
Drug Interactions
Digoxin
Increased serum digoxin levels.
Antacids
Decreased peak plasma levels of azithromycin.
Warfarin
Increased anticoagulant effect.
Nelfinavir
Increased azithromycin concentrations.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe nausea, vomiting, and diarrhea. Management typically involves gastric lavage and general supportive measures as clinically indicated.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use during pregnancy only if clearly needed. Excreted in breast milk, use with caution during lactation.
