Description
Overview of the medicine
Axim 1.5 gm Injection contains Cefuroxime Sodium, a second-generation cephalosporin antibiotic, used to treat a wide range of bacterial infections including respiratory tract infections, skin and soft tissue infections, urinary tract infections, and surgical prophylaxis.
Uses & Indications
Dosage
Adults
Adults: 750 mg - 1.5 gm every 8 hours for severe infections. For surgical prophylaxis, 1.5 gm IV pre-operatively, followed by 750 mg IV/IM every 8 hours for up to 24-48 hours.
Elderly
Elderly: Dosage as per adults, but monitor renal function and adjust if necessary.
Renal_impairment
Renal impairment: Adjust dose based on creatinine clearance. For CrCl 10-20 mL/min, 750 mg every 12 hours. For CrCl <10 mL/min, 750 mg every 24 hours.
How to Take
Axim 1.5 gm Injection should be administered intravenously (IV) or intramuscularly (IM) after reconstitution with appropriate diluents (e.g., Water for Injection). For IV administration, inject slowly over 3-5 minutes or infuse over 30-60 minutes. For IM administration, inject deep into a large muscle mass.
Mechanism of Action
Cefuroxime acts by inhibiting bacterial cell wall synthesis, primarily by binding to penicillin-binding proteins (PBPs) located inside the bacterial cell wall. This binding prevents the cross-linking of peptidoglycan units, which are essential for the integrity and rigidity of the bacterial cell wall, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Rapid onset of action, typically within minutes for IV administration and 15-60 minutes for IM administration.
Excretion
Excreted primarily by the kidneys via glomerular filtration and tubular secretion.
Half life
Plasma half-life is approximately 1 to 1.5 hours in adults with normal renal function.
Absorption
Rapidly absorbed after intramuscular (IM) or intravenous (IV) administration. Peak plasma concentrations are attained within 15-60 minutes after IM injection.
Metabolism
Cefuroxime is not significantly metabolized in the body. It is excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cefuroxime or any other cephalosporin antibiotics
- History of severe hypersensitivity reaction to any beta-lactam antibiotic (e.g., penicillins)
Drug Interactions
Probenecid
May increase cefuroxime levels and prolong its effect by reducing renal tubular secretion.
Oral anticoagulants
May enhance the anticoagulant effect, requiring closer monitoring of INR.
Storage
Store dry powder below 30°C, protected from light and moisture. Reconstituted solution should be used immediately; if not, store in a refrigerator (2-8°C) for up to 24 hours.
Overdose
Symptoms of overdose include neurological sequelae such as encephalopathy, convulsions, and coma, especially in patients with renal impairment. Treatment is symptomatic and supportive. Hemodialysis can aid in the removal of cefuroxime from the body.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy if clearly needed. Excreted in breast milk in small amounts; generally considered safe, but monitor infant for potential side effects (e.g., diarrhea, candidiasis).

Doctor Notes
- Ensure patient is not allergic to cephalosporins or penicillins before administration.
- Monitor renal function, especially in elderly or patients with pre-existing kidney conditions.
- Culture and susceptibility testing should be performed when appropriate.
Patient Guidelines
- Complete the full course of antibiotics even if symptoms improve.
- Report any severe or persistent side effects to your doctor.
- Inform your doctor about all other medications you are taking.
Missed Dose Advice
If a dose is missed, administer it as soon as remembered, unless it is almost time for the next dose. Do not double the dose to catch up.
