Description
Overview of the medicine
Axim 125 mg Tablet contains Cefuroxime Axetil, a second-generation cephalosporin antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Typical dose for mild to moderate infections is 250 mg twice daily. For 125 mg, often used for children or specific less severe adult infections, dosage must be prescribed by a physician. For pharyngitis/tonsillitis: 125 mg twice daily for 5-10 days.
Elderly
No special dosage adjustment is required unless there is severe renal impairment. Follow adult dosage.
Renal_impairment
Dosage should be adjusted based on creatinine clearance (CrCl). For CrCl 10-29 mL/min, 125-250 mg once daily. For CrCl <10 mL/min, 125-250 mg every 48 hours.
How to Take
Take the tablet orally with food for better absorption and to reduce gastrointestinal upset. Swallow whole, do not crush or chew.
Mechanism of Action
Cefuroxime Axetil is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) which interferes with the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 2-3 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine by glomerular filtration and tubular secretion.
Half life
Approximately 1-1.5 hours.
Absorption
Approximately 37-52% absorbed from the gastrointestinal tract, increased when taken with food.
Metabolism
Hydrolyzed by esterases in the intestinal mucosa and blood to release active cefuroxime. Not extensively metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to cefuroxime, any other cephalosporin antibiotics, or any component of the formulation.
- History of severe hypersensitivity reaction to any other beta-lactam antibiotics (e.g., penicillins).
Drug Interactions
Probenecid
Increases plasma concentration and prolongs the half-life of cefuroxime.
Oral Contraceptives
May reduce the efficacy of oral contraceptives. Additional non-hormonal contraception is recommended.
Antacids/H2-blockers/PPIs
May decrease the bioavailability of cefuroxime axetil by reducing gastric acidity. Cefuroxime axetil should be taken at least 2 hours before or several hours after these agents.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include convulsions and other neurological disorders. Management is supportive, and hemodialysis may be effective in reducing serum cefuroxime levels.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of impaired fertility or harm to the fetus. Use during pregnancy only if clearly needed. Cefuroxime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
