Overview of the medicine
Axim 500 mg Tablet contains Cefuroxime Axetil, a second-generation cephalosporin antibiotic, used to treat a wide range of bacterial infections including respiratory tract infections, urinary tract infections, skin and soft tissue infections, and otitis media.
Uses & Indications
Dosage
Adults
Most infections: 250 mg twice daily for 7-10 days. More severe infections (e.g., bronchitis, pneumonia): 500 mg twice daily for 7-10 days. Uncomplicated UTI: 250 mg twice daily for 3-7 days. Gonorrhea: 1000 mg as a single dose.
Elderly
Dosage similar to adults, but adjust for renal function.
Renal_impairment
Reduce dosage interval based on creatinine clearance. For CrCl 10-29 mL/min, administer standard dose every 24 hours. For CrCl <10 mL/min, administer standard dose every 48 hours.
How to Take
Oral. For optimal absorption, Axim 500 mg tablet should be taken with food. Do not crush or chew the tablet; swallow whole.
Mechanism of Action
Cefuroxime Axetil works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located in the bacterial cell wall, thereby disrupting the formation of peptidoglycan, an essential component of the bacterial cell wall. This leads to cell lysis and bacterial death.

Excretion
Mainly excreted unchanged in the urine by glomerular filtration and tubular secretion.
Half life
Approximately 1.2 to 1.5 hours in adults.
Absorption
Well absorbed from the gastrointestinal tract and rapidly hydrolyzed to cefuroxime. Enhanced absorption with food.
Metabolism
Not significantly metabolized; mainly hydrolyzed to cefuroxime in the intestinal mucosa and blood.
Side Effects
Contraindications
- Known hypersensitivity to cefuroxime, any other cephalosporin antibiotic, or to any of the excipients.
- History of severe hypersensitivity to any other type of beta-lactam antibacterial agent (e.g., penicillins).
Drug Interactions
Probenecid
Increases cefuroxime levels by decreasing renal tubular secretion, potentially increasing toxicity.
Oral Contraceptives
May decrease efficacy of oral contraceptives. Advise using additional birth control.
Antacids/H2 Blockers/PPIs
May reduce bioavailability of cefuroxime axetil, especially if taken together. Separate administration times.
Aminoglycosides/Potent Diuretics
Concurrent use with cefuroxime may increase nephrotoxicity, monitor renal function.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include convulsions and other neurological disorders. Management involves symptomatic and supportive care. Hemodialysis and peritoneal dialysis can effectively remove cefuroxime from the body.

Pregnancy category B. Studies in animals have not shown harm to the fetus, but no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Excreted in breast milk in small amounts; use with caution in breastfeeding mothers. Monitor infants for diarrhea or candidiasis.
