Cefaclav 125 mg Tablet is an antibiotic belonging to the cephalosporin group, combined with a beta-lactamase inhibitor. It is used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
Typically, higher strengths (e.g., 250 mg or 500 mg Cefaclor equivalent) are preferred for adults, usually 250-500 mg Cefaclor every 8 hours. This 125 mg tablet is generally for pediatric use.
Elderly
Similar to adult dosage; monitor renal function. Dosage adjustments may be needed based on creatinine clearance.
Renal_impairment
For CrCl 10-50 mL/min, administer usual dose less frequently. For CrCl < 10 mL/min, administer usual dose every 12-24 hours.
How to Take
Oral. Can be taken with or without food. To minimize gastrointestinal upset, it may be taken with food.
Mechanism of Action
Cefaclor inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis. Clavulanic acid is a beta-lactamase inhibitor that prevents bacterial enzymes from inactivating Cefaclor, thereby extending its spectrum of activity against beta-lactamase producing bacteria.

Excretion
Primarily excreted unchanged by the kidneys (Cefaclor). Clavulanic acid is excreted renally and non-renally.
Half life
Approximately 0.5 to 1 hour for Cefaclor, Clavulanic acid is also short.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentrations are achieved within 0.5 to 1 hour.
Metabolism
Cefaclor is minimally metabolized. Clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- Hypersensitivity to cefaclor, clavulanic acid, any other cephalosporin, or any component of the formulation.
- History of severe hypersensitivity reaction to any other beta-lactam antibacterial agent (e.g., penicillins, carbapenems).
Drug Interactions
Warfarin
May increase anticoagulant effect. Monitor INR.
Probenecid
Increases and prolongs plasma concentrations of cefaclor.
Aminoglycosides
Possible increased nephrotoxicity.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, epigastric distress, and diarrhea. Management includes general supportive measures; gastric lavage or activated charcoal may be considered. Hemodialysis is not effective.
