Description
Overview of the medicine
Cefaclav-250-mg-tablet is an oral antibiotic medication comprising a third-generation cephalosporin, Cefixime, combined with Clavulanic Acid, a beta-lactamase inhibitor. This combination provides enhanced antibacterial activity against a broad spectrum of Gram-positive and Gram-negative bacteria, including those resistant to Cefixime alone due to beta-lactamase production. It is used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Usually Cefixime 200 mg + Clavulanic Acid 125 mg, orally twice daily for 7-14 days, depending on the infection severity and type.
Elderly
No specific dose adjustment is usually required unless severe renal impairment is present. Monitor renal function.
Renal_impairment
Dose adjustment required for creatinine clearance < 30 mL/min. Consult physician for specific dosing.
How to Take
Take orally with or without food. To minimize gastrointestinal upset, it is often recommended to take with food.
Mechanism of Action
Cefixime, a cephalosporin, inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial lysis and death. Clavulanic Acid, a beta-lactamase inhibitor, inactivates bacterial beta-lactamase enzymes, which would otherwise degrade Cefixime, thereby extending Cefixime's spectrum of activity.
Pharmacokinetics
Onset
Within 2-4 hours
Excretion
Cefixime is primarily excreted unchanged in urine. Clavulanic Acid is excreted via urine, partially as metabolites.
Half life
Cefixime: 3-4 hours; Clavulanic Acid: approximately 1 hour
Absorption
Cefixime is approximately 40-50% absorbed orally. Clavulanic Acid is well absorbed orally. Food may delay absorption but does not significantly affect overall bioavailability.
Metabolism
Cefixime is not significantly metabolized. Clavulanic Acid is extensively metabolized.
Side Effects
Contraindications
- Hypersensitivity to cefixime, clavulanic acid, other cephalosporins, penicillins, or any component of the formulation.
- History of severe hypersensitivity reaction to any other beta-lactam antibacterial agent.
Drug Interactions
Probenecid
May increase Cefixime plasma concentrations.
Oral Contraceptives
May reduce the efficacy of oral contraceptives; advise additional barrier methods.
Anticoagulants (e.g., Warfarin)
May prolong prothrombin time (PT) and INR. Monitor coagulation parameters.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Gastric lavage may be indicated. Hemodialysis or peritoneal dialysis may remove some drug. Symptomatic and supportive treatment is recommended.
Pregnancy & Lactation
Use during pregnancy only if clearly needed. Cefixime is generally considered safe in pregnancy (Category B). Clavulanic Acid is also Category B. Both are excreted in breast milk in small amounts; use with caution during lactation.

Lab Monitoring
- Renal function tests (e.g., creatinine, BUN) in patients with impaired renal function.
- Liver function tests (ALT, AST) if prolonged use or pre-existing hepatic impairment.
- Complete blood count (CBC) during prolonged therapy.
Doctor Notes
- Confirm bacterial infection before prescribing; not effective against viral infections.
- Consider culture and sensitivity testing to guide therapy, especially in recurrent or severe infections.
- Advise patients on potential gastrointestinal side effects and the importance of completing the full course.
Patient Guidelines
- Complete the full course of treatment as prescribed, even if symptoms improve.
- Do not share this medication with others.
- Inform your doctor about any allergies or medical conditions before starting treatment.
Missed Dose Advice
Take the missed dose as soon as you remember, unless it is almost time for your next dose. Do not double doses.
Driving Precautions
May cause dizziness in some patients; use caution when driving or operating machinery until you know how the medication affects you.
