Overview of the medicine
Cefamax is a brand name for Cefixime, a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
200 mg to 400 mg once daily or in two divided doses. For most infections, 400 mg once daily is recommended. For uncomplicated gonorrhea, a single oral dose of 400 mg.
Elderly
No dosage adjustment is generally required unless renal function is significantly impaired.
Renal_impairment
For creatinine clearance 21-60 mL/min, 75% of standard dose. For creatinine clearance <20 mL/min, 50% of standard dose.
How to Take
Can be taken with or without food. For suspension, reconstitute with boiled and cooled water as directed and shake well before use. Complete the full course of treatment as prescribed, even if symptoms improve.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis, leading to cell lysis and bacterial death. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby interfering with the transpeptidation reaction and preventing the cross-linking of peptidoglycan chains, which is essential for cell wall strength and rigidity.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved in 2-6 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine (approximately 50-55% of absorbed dose) via glomerular filtration and tubular secretion. A small amount is excreted in bile.
Half life
Average plasma half-life is 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food slows absorption but does not significantly alter the extent of absorption.
Metabolism
Not extensively metabolized in the body. Primary route of elimination is renal.
Side Effects
Contraindications
- •Known hypersensitivity to cefixime or any other cephalosporin antibiotics

Drug Interactions
Probenecid
May increase cefixime plasma levels by decreasing renal excretion.
Carbamazepine
Increased plasma levels of carbamazepine have been reported with concomitant cefixime administration.
Oral Anticoagulants (e.g., Warfarin)
May increase prothrombin time; INR monitoring is recommended.
Storage
Store in a cool, dry place, below 30°C, away from direct sunlight and moisture. Keep out of reach of children. Reconstituted suspension should be stored in a refrigerator (2°C-8°C) and discarded after the specified period.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Should be used during pregnancy only if clearly needed. Excreted in small amounts in breast milk; caution should be exercised when administered to a nursing woman.
