Overview of the medicine
Cefixime is a third-generation cephalosporin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Adults and children >10 years or >50 kg: 200-400 mg once daily or 200 mg twice daily. Duration depends on infection type, typically 7-14 days.
Elderly
No specific dose adjustment needed unless renal impairment is present.
Renal_impairment
CrCl 21-60 mL/min: 75% of standard dose. CrCl <20 mL/min: 50% of standard dose.
How to Take
Can be taken with or without food. For better absorption and to reduce gastrointestinal upset, it is often recommended to take it with food.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan units, leading to cell lysis and bacterial death.

Onset
Peak plasma concentrations reached within 2-6 hours.
Excretion
Primarily by renal excretion (glomerular filtration and tubular secretion).
Half life
Average plasma half-life is 3-4 hours.
Absorption
Approximately 40-50% orally absorbed.
Metabolism
Not significantly metabolized; mainly excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to cefixime or other cephalosporins
- •History of anaphylactic reaction to penicillin
Drug Interactions
Warfarin
May increase prothrombin time and INR, increasing risk of bleeding.
Nifedipine
Increases cefixime absorption.
Carbamazepine
Increased carbamazepine levels.
Oral anticoagulants
Enhanced anticoagulant effect.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to increased side effects, especially gastrointestinal upset. Treatment is supportive, and hemodialysis/peritoneal dialysis are not effective in removing cefixime.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy if clearly needed. Excreted in breast milk in small amounts; use with caution during lactation.
