Cefuroxime Axetil is a second-generation cephalosporin antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Typical dosage is 250 mg to 500 mg twice daily, depending on the severity and type of infection. For uncomplicated urinary tract infections, 250 mg twice daily. For most other infections, 500 mg twice daily.
Elderly
No specific dose adjustment is required solely based on age; however, renal function should be assessed and dosage adjusted if renal impairment is present.
Renal_impairment
Dose reduction is necessary for severe renal impairment (e.g., creatinine clearance < 30 mL/min). For example, 250 mg once daily or 250 mg every 24 hours for severe impairment.
How to Take
Take orally with food to enhance absorption and reduce gastrointestinal upset. Swallow the tablet whole; do not crush or chew.
Mechanism of Action
Cefuroxime Axetil works by inhibiting bacterial cell wall synthesis, leading to bacterial cell death (bactericidal action).

Excretion
Primarily excreted unchanged by glomerular filtration and tubular secretion in the urine.

Metabolism
Cefuroxime itself is not extensively metabolized in the liver.
Side Effects
Contraindications
- •Known hypersensitivity to cefuroxime, any other cephalosporin antibiotics, or any component of the formulation.
- •Previous severe hypersensitivity reaction to any other type of beta-lactam antibacterial agents (e.g., penicillins, monobactams, carbapenems).
Drug Interactions
Probenecid
Concomitant use with probenecid increases cefuroxime concentrations and prolongs its duration of action.
Oral Contraceptives
Cefuroxime may reduce the efficacy of oral contraceptives. Advise patients to use additional barrier contraception.
Warfarin and other oral anticoagulants
Concurrent use may increase the anticoagulant effect of warfarin. Monitor INR closely.
Antacids/H2-receptor antagonists/Proton pump inhibitors
May reduce the bioavailability of cefuroxime axetil by increasing gastric pH.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include gastrointestinal disturbances such as nausea, vomiting, and diarrhea. If an overdose occurs, seek immediate medical attention. Management is supportive and symptomatic; hemodialysis may be considered.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. Use during pregnancy only if clearly needed. Cefuroxime is excreted into breast milk in small amounts; generally considered safe for breastfeeding infants, but use with caution and monitor for side effects in the infant.
