Overview of the medicine
Cefixime is a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
The usual adult dose is 200 mg once daily or 100 mg twice daily. For more severe infections or uncomplicated gonorrhea, a single 400 mg dose may be given once daily.
Elderly
No specific dose adjustment is needed for elderly patients unless there is significant renal impairment.
Renal_impairment
Dosage should be adjusted based on creatinine clearance (CrCl). For CrCl 21-60 mL/min, 200 mg daily. For CrCl < 20 mL/min, 100 mg daily.
How to Take
Take orally, with or without food. It is recommended to take at the same time each day for best results. Swallow the capsule whole with water.
Mechanism of Action
Cefixime exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), leading to the disruption of peptidoglycan synthesis.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved in 2-6 hours.
Excretion
Primarily excreted by the kidneys, with about 50% of an absorbed dose excreted unchanged in the urine within 24 hours. Non-renal excretion accounts for the remainder.
Half life
Plasma half-life is typically 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food may delay absorption but does not significantly affect the extent of absorption.
Metabolism
Not extensively metabolized in the body. Some metabolites may be formed but are generally inactive.
Side Effects
Contraindications
- •Hypersensitivity to cefixime or any other cephalosporin antibiotics.
- •History of severe hypersensitivity (e.g., anaphylaxis) to any beta-lactam antibiotic (e.g., penicillins, carbapenems).
Drug Interactions
Carbamazepine
Concomitant use with cefixime has been reported to increase carbamazepine levels. Therapeutic drug monitoring may be necessary.
Warfarin and other oral anticoagulants
Cefixime may increase the prothrombin time (PT) and INR, potentially leading to an increased risk of bleeding. Close monitoring is recommended.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. Gastric lavage may be indicated. Hemodialysis or peritoneal dialysis is not effective in removing cefixime from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use in pregnancy only if clearly needed. Cefixime is excreted in small amounts in human milk; use with caution during lactation.
