Description
Overview of the medicine
Cefixime is a broad-spectrum cephalosporin antibiotic used to treat a wide range of bacterial infections in children, including respiratory tract, urinary tract, and ear infections.
Uses & Indications
Dosage
Adults
Not applicable for this pediatric formulation.
Elderly
Not applicable for this pediatric formulation.
Renal_impairment
For children with creatinine clearance (CrCl) <30 mL/min, dosage reduction is required. Consult a physician for specific adjustments.
How to Take
Administer orally, with or without food. Shake the bottle well before each use. Can be mixed with a small amount of water, milk, or formula immediately before administration.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), leading to the interruption of peptidoglycan synthesis, which ultimately causes cell lysis and bacterial death.
Pharmacokinetics
Onset
Peak plasma concentrations are usually achieved within 2-6 hours after oral administration.
Excretion
Approximately 50-60% of the absorbed dose is excreted unchanged in the urine over 24 hours. Biliary excretion is also a route of elimination.
Half life
The plasma elimination half-life is typically 3-4 hours, but can extend to 6-8 hours.
Absorption
Approximately 40-50% of an oral dose is absorbed. Absorption may be slightly delayed by food but the total amount absorbed is not significantly affected.
Metabolism
Cefixime is not extensively metabolized in the body. There are no active metabolites.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reaction (e.g., anaphylaxis) to any penicillin antibiotic.
Drug Interactions
Probenecid
Increases cefixime plasma concentrations and prolongs its elimination half-life by decreasing renal tubular secretion.
Carbamazepine
Concomitant use may increase carbamazepine levels; monitoring is advised.
Warfarin and other anticoagulants
May increase prothrombin time (INR), requiring close monitoring and possible anticoagulant dose adjustment.
Storage
Store dry powder below 30°C, protected from light and moisture. After reconstitution, store the suspension in a refrigerator (2-8°C) and discard any unused portion after the specified period (typically 14 days). Do not freeze.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not substantially removed by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Animal reproduction studies have not shown a risk. Use in pregnancy only if clearly needed. Cefixime is excreted in small amounts in breast milk; caution is advised when administered to a nursing mother.

Numerous clinical trials have confirmed the efficacy and safety of cefixime in the treatment of various bacterial infections in pediatric patients. Studies support its use for common respiratory, urinary, and ear infections.
Lab Monitoring
- Renal function monitoring in patients with pre-existing renal impairment or during prolonged therapy.
- Liver function tests (LFTs) periodically, especially in patients with hepatic dysfunction.
Doctor Notes
- Emphasize to parents/caregivers the importance of completing the full course of therapy to ensure eradication of infection and prevent resistance.
- Advise monitoring for signs of allergic reactions, severe diarrhea (potentially C. difficile-associated colitis), or superinfections during and after treatment.
Patient Guidelines
- Complete the full course of medication, even if symptoms improve, to prevent bacterial resistance and relapse.
- Shake the bottle well before each use and measure the dose accurately using the provided dropper.
- Do not share this medicine with others, even if they have similar symptoms.
Missed Dose Advice
If a dose is missed, administer it as soon as remembered. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Cefixime may cause dizziness in some individuals. Patients should be cautioned about operating machinery or driving if they experience dizziness.
Lifestyle Advice
- Maintain good hygiene practices to prevent the spread of infection.
- Ensure adequate hydration during illness.
