Overview of the medicine
Cefixime is an oral third-generation cephalosporin antibiotic used to treat various bacterial infections susceptible to Cefixime.
Uses & Indications
Dosage
Adults
The usual adult dose is 400 mg once daily or 200 mg every 12 hours. Treatment duration usually 7-14 days depending on infection type.
Elderly
No dosage adjustment is required in elderly patients with normal renal function. Monitor renal function.
Renal_impairment
For creatinine clearance 21-60 mL/min: 300 mg daily. For creatinine clearance <20 mL/min: 200 mg daily. Hemodialysis and CAPD patients: 200 mg daily.
How to Take
Cephoral 400 mg Capsule should be taken orally with or without food. It is absorbed well after oral administration.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which leads to cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 2-6 hours.
Excretion
Approximately 50% of an absorbed dose is excreted unchanged in the urine over 24 hours. Some biliary excretion occurs.
Half life
Plasma elimination half-life is typically 3-4 hours.
Absorption
Approximately 40-50% orally absorbed, influenced by food but not to a clinically significant extent.
Metabolism
Not extensively metabolized in the liver; limited metabolism has been observed.
Side Effects
Contraindications
- Hypersensitivity to Cefixime or other cephalosporins.
- History of hypersensitivity to penicillin (due to potential cross-reactivity).
Drug Interactions
Warfarin
Concurrent administration may prolong prothrombin time (INR).
Nifedipine
Concurrent use may increase Cefixime absorption and peak plasma levels.
Carbamazepine
Elevated carbamazepine levels have been reported with concurrent use.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Supportive care is recommended.

Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in breast milk; use with caution during lactation.
