Description
Overview of the medicine
Cerox-A 125 mg Suspension is an oral antibiotic used to treat a wide range of bacterial infections. It contains Cefuroxime Axetil, a second-generation cephalosporin, effective against various susceptible bacteria.
Uses & Indications
Dosage
Adults
Not typically used for adults in 125mg suspension strength; adults usually take higher strength tablets.
Children
Children (3 months - 12 years): 125 mg twice daily for most infections (e.g., tonsillitis, pharyngitis, sinusitis), or 15 mg/kg twice daily (up to 250 mg twice daily) for severe infections like otitis media.
Renal_impairment
Dosage adjustment required based on creatinine clearance.
How to Take
Administer orally. Shake the bottle well before each use. Can be taken with or without food, but taking with food may enhance absorption and reduce gastrointestinal discomfort.
Mechanism of Action
Cefuroxime Axetil is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), leading to the disruption of bacterial cell wall formation and subsequent cell lysis.
Pharmacokinetics
Onset
Peak plasma concentrations typically occur 2-3 hours after administration.
Excretion
Primarily excreted unchanged in the urine by glomerular filtration and tubular secretion.
Half life
Approximately 1.2 to 1.5 hours.
Absorption
Well absorbed from the gastrointestinal tract and rapidly hydrolyzed to cefuroxime. Food enhances absorption.
Metabolism
Cefuroxime itself is not metabolized in the body. The axetil ester is hydrolyzed in the intestinal mucosa and blood.
Side Effects
Contraindications
- Hypersensitivity to cefuroxime, other cephalosporins, or any component of the formulation
- History of severe hypersensitivity reactions to any other type of beta-lactam antibacterial agent (e.g., penicillins)
Drug Interactions
Probenecid
Increases cefuroxime plasma concentrations and prolongs its half-life by reducing renal excretion.
Oral Contraceptives
May reduce the efficacy of oral contraceptives (advise additional contraceptive measures).
Aminoglycosides/Loop Diuretics
Increased risk of nephrotoxicity with concomitant use, especially in patients with impaired renal function.
Antacids/H2 blockers/Proton Pump Inhibitors
May decrease the bioavailability of cefuroxime axetil by increasing gastric pH. Administer at least 1 hour before or several hours after these agents.
Storage
Store dry powder below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children. Reconstituted suspension should be stored in a refrigerator (2-8°C) and discarded after 10 days.
Overdose
Symptoms of overdose include cerebral irritation leading to convulsions. Management is symptomatic and supportive. Hemodialysis and peritoneal dialysis may be considered to remove the drug.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of impaired fertility or harm to the fetus. Cefuroxime is excreted in human milk in low concentrations; caution should be exercised when administered to a nursing mother.

Lab Monitoring
- Periodic assessment of renal, hepatic, and hematopoietic functions during prolonged therapy.
- Monitor for signs of superinfection.
- Coombs' test may be positive, which could interfere with cross-matching blood.
Doctor Notes
- Consider renal function when dosing, especially in pediatric patients with developing kidneys or adults with impairment.
- Advise patients on the importance of completing the full course of therapy.
- Monitor for signs of allergic reaction, especially in patients with penicillin allergy history.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor.
- Complete the full course of treatment, even if you feel better, to prevent recurrence and antibiotic resistance.
- Shake the suspension well before each use.
- Report any severe or persistent side effects to your doctor immediately.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
