Description
Overview of the medicine
Cefuroxime Axetil is a broad-spectrum cephalosporin antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
250 mg or 500 mg twice daily, depending on the severity and type of infection. For most infections, 250 mg twice daily. For severe infections or lower respiratory tract infections, 500 mg twice daily.
Elderly
No specific dose adjustment is generally needed unless severe renal impairment is present.
Renal_impairment
Dose adjustment required for severe renal impairment (creatinine clearance < 30 mL/min). Consult a physician for specific dosing.
How to Take
Administer orally, preferably with food for optimal absorption and to reduce gastrointestinal discomfort. Do not crush or chew the tablet.
Mechanism of Action
Inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Typically within 1-2 hours after oral administration.
Excretion
Primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion.
Half life
Approximately 1 to 1.5 hours.
Absorption
Rapidly absorbed after oral administration and hydrolyzed to cefuroxime in the intestinal mucosa and blood. Bioavailability is increased when taken with food.
Metabolism
Not extensively metabolized in the body; primarily undergoes hydrolysis.
Side Effects
Contraindications
- Known hypersensitivity to cefuroxime, any other cephalosporin antibiotics, or any component of the formulation.
- History of severe hypersensitivity (e.g., anaphylaxis) to any other type of beta-lactam antibacterial agent (e.g., penicillins, carbapenems, monobactams).
Drug Interactions
Probenecid
Concomitant administration with probenecid increases the area under the serum concentration-time curve of cefuroxime by about 50%, prolonging its half-life.
Oral Contraceptives
Cefuroxime may affect the gut flora, leading to lower estrogen reabsorption and reduced efficacy of combined oral contraceptives. Advise additional contraception.
Aminoglycosides and Diuretics
Concurrent use of high doses of cephalosporins with aminoglycosides or potent diuretics (e.g., furosemide) may increase the risk of nephrotoxicity.
Antacids/H2-receptor antagonists
Drugs that reduce gastric acidity may reduce the bioavailability of cefuroxime axetil. It is recommended to take cefuroxime axetil at least 2 hours before or after antacids.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose of cephalosporins can cause cerebral irritation leading to convulsions. Management is primarily symptomatic and supportive. Hemodialysis may be used to aid in the removal of cefuroxime from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of impaired fertility or harm to the fetus. However, use during pregnancy only if clearly needed. Cefuroxime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing woman.
