Overview of the medicine
Cerox-A 750 mg Injection contains Cefuroxime, a second-generation cephalosporin antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Usually 750 mg three times daily (every 8 hours) by IM or IV injection. For severe infections, the dose may be increased to 1.5 g three times daily. For surgical prophylaxis, 1.5 g IV 30-60 minutes before surgery, followed by 750 mg IM/IV every 8 hours for up to 24-48 hours.
Elderly
No specific dose reduction required in elderly patients with normal renal function. Dose should be adjusted based on renal function if impaired.
Renal_impairment
Dosage adjustment is necessary. For creatinine clearance 10-20 mL/min, 750 mg twice daily. For creatinine clearance <10 mL/min, 750 mg once daily. For hemodialysis patients, 750 mg after each dialysis session.
How to Take
For intramuscular administration, dissolve the powder in sterile water for injection. For intravenous administration, reconstitute the powder and then dilute with a suitable intravenous fluid and administer over 3 to 5 minutes or infuse over 30 to 60 minutes.
Mechanism of Action
Cefuroxime is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.

Mainly excreted unchanged by glomerular filtration and tubular secretion via the kidneys.

Approximately 1-1.5 hours in adults with normal renal function.

Rapidly absorbed after intramuscular (IM) administration, reaching peak plasma concentrations within 30-60 minutes. Intravenous (IV) administration results in immediate peak concentrations.

Side Effects
Contraindications
- Known hypersensitivity to cefuroxime, any other cephalosporins, or to any of the excipients.
- History of severe hypersensitivity (e.g., anaphylactic reaction) to any other type of beta-lactam antibacterial agent (e.g., penicillins, carbapenems, monobactams).
Drug Interactions
Probenecid
Concomitant administration with probenecid increases cefuroxime serum concentrations and prolongs its half-life.
Oral Anticoagulants
Concomitant use may increase the anticoagulant effect.
Nephrotoxic drugs (e.g., aminoglycosides, loop diuretics)
Concurrent administration with potentially nephrotoxic drugs may increase the risk of renal toxicity.
Storage
Store below 30°C in a dry place, protected from light. Keep out of reach of children. Reconstituted solution should be used immediately or stored for a short period as directed.

Overdose can lead to neurological sequelae including convulsions. Management is symptomatic and supportive. Hemodialysis and peritoneal dialysis can reduce serum levels.
