Description
Overview of the medicine
Cerox-CV 250 mg Tablet is a combination antibiotic containing cefuroxime and clavulanic acid, used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Usually 250 mg twice daily for 7-10 days, or as directed by physician. For more severe infections, 500 mg twice daily may be prescribed.
Elderly
Same as adult dosage, but caution with renal impairment and dose adjustment may be necessary.
Renal_impairment
Dosage adjustment required based on creatinine clearance. For CrCl < 30 mL/min, dosage reduction is advised.
How to Take
Orally, preferably with food for optimum absorption and to minimize gastrointestinal discomfort. Do not crush or chew the tablet.
Mechanism of Action
Cefuroxime is a second-generation cephalosporin antibiotic that inhibits bacterial cell wall synthesis. Clavulanic acid is a beta-lactamase inhibitor that prevents bacterial enzymes from inactivating cefuroxime, thereby extending its antibacterial spectrum against beta-lactamase producing strains.
Pharmacokinetics
Onset
Peak plasma concentrations are generally achieved within 1-2 hours after oral administration.
Excretion
Both cefuroxime and clavulanic acid are primarily excreted unchanged or as metabolites in the urine, mainly by glomerular filtration and tubular secretion.
Half life
Cefuroxime: approximately 1.2-1.5 hours; Clavulanic acid: approximately 1 hour.
Absorption
Cefuroxime axetil is well absorbed from the gastrointestinal tract and rapidly hydrolyzed to cefuroxime. Absorption is increased when taken with food. Clavulanic acid is also well absorbed.
Metabolism
Cefuroxime is not metabolized in the body. Clavulanic acid is extensively metabolized and then eliminated.
Side Effects
Contraindications
- Hypersensitivity to cefuroxime, clavulanic acid, or any other cephalosporin antibiotics.
- History of severe hypersensitivity reaction to any other beta-lactam antibacterial agents (e.g., penicillins).
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin. INR monitoring is advised.
Probenecid
Concomitant use with probenecid increases and prolongs the serum concentrations of cefuroxime.
Oral Contraceptives
May reduce the efficacy of oral contraceptives. Additional non-hormonal contraception methods should be considered.
Antacids/H2 blockers
May decrease the bioavailability of cefuroxime axetil, potentially reducing its effectiveness.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Overdose symptoms may include gastrointestinal disturbances such as nausea, vomiting, diarrhea, and central nervous system effects including convulsions. Management is symptomatic and supportive. Hemodialysis may be considered in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. Use during pregnancy only if clearly needed. Use with caution during lactation, as small amounts of cefuroxime are excreted in breast milk.
